Th<strong>is</strong> multid<strong>is</strong>cipl<strong>in</strong>ary team <strong>is</strong> <strong>in</strong>volved dur<strong>in</strong>g the assessment as well as dur<strong>in</strong>g therapy.<br />
In some pathways a psycholog<strong>is</strong>t <strong>is</strong> <strong>in</strong>volved. In half of the pathways a social worker <strong>is</strong><br />
<strong>in</strong>volved. Only <strong>in</strong> 2 of the pathways the <strong>in</strong>volvement of a special<strong>is</strong>t <strong>in</strong> rehabilitation <strong>is</strong><br />
explicitly mentioned.<br />
7.6.2.7 Intensity of therapy<br />
Intensity of therapy was never mentioned.<br />
7.6.2.8 Type of therapy<br />
In the cases where the start of rehabilitation therapy <strong>is</strong> <strong>in</strong>cluded <strong>in</strong> the pathway for the<br />
acute phase, it concerns always physical therapy, occupational therapy and speech<br />
therapy.<br />
Type of therapy or <strong>in</strong>volved professionals are not def<strong>in</strong>ed <strong>in</strong> one pathway for the postacute<br />
phase. Instead of th<strong>is</strong>, specific <strong>in</strong>tervention goals <strong>in</strong> different doma<strong>in</strong>s are l<strong>is</strong>ted. An<br />
example for the doma<strong>in</strong> of activities of daily liv<strong>in</strong>g: patient has basel<strong>in</strong>e skills <strong>in</strong> feed<strong>in</strong>g,<br />
hygiene/groom<strong>in</strong>g and dress<strong>in</strong>g. Th<strong>is</strong> makes it easier to implement th<strong>is</strong> pathway <strong>in</strong><br />
different organ<strong>is</strong>ations <strong>with</strong> typical activities per type of professional. <strong>The</strong> other pathway<br />
for the post-acute phase conta<strong>in</strong>s specific goals directly l<strong>in</strong>ked to a type of professional.<br />
Goals change over the four phases. In each phase physical therapy, occupational therapy<br />
and speech therapy are offered.<br />
7.6.2.9 Expected outcomes and the use of outcome measurement tools<br />
Based on the results of the assessment, treatment goals are identified. In one of the<br />
pathways it was expla<strong>in</strong>ed that goals were determ<strong>in</strong>ed by stroke severity, number and<br />
degree of impairments, expected outcome, pre-morbid functional status, and<br />
patient/caregiver attributes or needs. For each therapeutic <strong>in</strong>tervention short term<br />
goals and estimated time to achieve these goals were def<strong>in</strong>ed <strong>in</strong> advance.<br />
Only <strong>in</strong> two pathways the same assessment tool <strong>is</strong> used at the beg<strong>in</strong>n<strong>in</strong>g and at the end<br />
of the pathway.<br />
In one pathway four categories of outcomes are identified:<br />
• Patient d<strong>is</strong>charge outcomes<br />
• Patient cl<strong>in</strong>ical outcomes<br />
• Patient sat<strong>is</strong>faction outcomes<br />
• F<strong>in</strong>al Program outcomes<br />
Assessment of cl<strong>in</strong>ical outcomes <strong>is</strong> used to determ<strong>in</strong>e rehabilitation goals and<br />
performed by use of:<br />
• National Institute of Health Stroke Scale Measures (Stroke deficits)<br />
• Orp<strong>in</strong>gton Prognostic Scale Measures (Stroke severity)<br />
• Barthel Index; FIM; Instrumental Activities of Daily Liv<strong>in</strong>g (ADL)<br />
• Fugl-Meyer; Duke Mobility Scale (Motor function)<br />
• Geriatric depression scale (Depression)
