7 CLINICAL PATHWAYS FOR<br />
REHABILITATION<br />
7.1 INTRODUCTION<br />
A limited research on rehabilitation practice <strong>in</strong> Belgium (See chapter 6) <strong>in</strong>dicated that a<br />
large variability <strong>is</strong> possible related to <strong>in</strong>tensity and duration of therapy for patients <strong>with</strong><br />
comparable character<strong>is</strong>tics. An additional question to f<strong>in</strong>d out whether th<strong>is</strong> variability <strong>is</strong><br />
also represented <strong>in</strong> the currently used cl<strong>in</strong>ical pathways, was formulated. <strong>The</strong> goal of<br />
th<strong>is</strong> chapter was an identification and compar<strong>is</strong>on of cl<strong>in</strong>ical pathways for rehabilitation<br />
after amputation of lower extremity, multiple scleros<strong>is</strong>, sp<strong>in</strong>al cord <strong>in</strong>jury, stroke and<br />
total hip replacement. Guidel<strong>in</strong>es and the results of random<strong>is</strong>ed cl<strong>in</strong>ical trials were<br />
considered as out of scope for th<strong>is</strong> chapter, as they are not necessarily reflected <strong>in</strong><br />
cl<strong>in</strong>ical practice.<br />
Due to a lack of scientific <strong>report</strong>s conta<strong>in</strong><strong>in</strong>g evidence to support all cl<strong>in</strong>ical dec<strong>is</strong>ions <strong>in</strong><br />
rehabilitation, the content of a cl<strong>in</strong>ical pathway <strong>is</strong> often <strong>in</strong>fluenced by the op<strong>in</strong>ion of<br />
stakeholders and the f<strong>in</strong>anc<strong>in</strong>g and organ<strong>is</strong>ational aspects of the health care system <strong>in</strong> a<br />
country. A compar<strong>is</strong>on of cl<strong>in</strong>ical pathways developed <strong>in</strong> different countries, might<br />
neutral<strong>is</strong>e these <strong>in</strong>fluences and might result <strong>in</strong> a more neutral description of the<br />
‘optimal’ cl<strong>in</strong>ical pathway.<br />
At a macroscopic level, the cl<strong>in</strong>ical pathways can be used to d<strong>is</strong>cuss current variability <strong>in</strong><br />
rehabilitation practice <strong>in</strong> Belgium. Besides, these cl<strong>in</strong>ical pathways can be used to argue<br />
certa<strong>in</strong> choices related to the future supply of rehabilitation services.<br />
Th<strong>is</strong> chapter was performed by a collaboration between the “Centrum voor<br />
Ziekenhu<strong>is</strong>- en Verpleg<strong>in</strong>gswetenschap, Leuven” (CZV) and the department of Physical<br />
Medic<strong>in</strong>e and Rehabilitation, Leuven (PMR).<br />
<strong>The</strong> CZV offered an <strong>in</strong>troduction course on cl<strong>in</strong>ical pathways and cont<strong>in</strong>uous support<br />
dur<strong>in</strong>g th<strong>is</strong> (sub-)project. <strong>The</strong> CZV provided a network of contacts concerned <strong>with</strong> the<br />
development of cl<strong>in</strong>ical pathways, <strong>in</strong> different countries.<br />
PMR composed a team of residents of Physical Medic<strong>in</strong>e and Rehabilitation.<br />
Cl<strong>in</strong>ical pathways were searched, analysed and compared by use of a general template<br />
(Figure 7.1) <strong>in</strong> which the most relevant parameters were <strong>in</strong>tegrated. It was not the<br />
objective to analyse the type of <strong>in</strong>terventions <strong>in</strong> detail.
