an outcome model but the application of ICF <strong>in</strong> cl<strong>in</strong>ical practice and for f<strong>in</strong>anc<strong>in</strong>g<br />

purposes only fits long term v<strong>is</strong>ion.<br />

Outcome measures, outcome models and patient classification systems ex<strong>is</strong>t but are<br />

not explicitly l<strong>in</strong>ked.<br />

<strong>The</strong> implementation of ICF as an outcome model depends on its compatibility <strong>with</strong><br />

measures used <strong>in</strong> rehabilitation and the improvement of its applicability.<br />

FIM and Barthel Index are tools measur<strong>in</strong>g level of dependence related to activities of<br />

daily liv<strong>in</strong>g. <strong>The</strong> results of th<strong>is</strong> measurement can be used to estimate workload, but<br />

neither FIM nor Barthel Index measure rehabilitation needs.<br />

In Belgium only MVG-RIM2 <strong>in</strong>cludes functional items. It could be considered to use the<br />

MVG-RIM2 as an <strong>in</strong>termediate profil<strong>in</strong>g tool of post-acute rehabilitation, us<strong>in</strong>g the<br />

“f<strong>in</strong>gerpr<strong>in</strong>ts”. However, the necessary reluctance <strong>is</strong> needed: MVG-RIM <strong>is</strong> not a tool<br />

enabl<strong>in</strong>g to score for paramedical (occupational therapy, physical therapy,<br />

psychology,…) activities, neither to monitor or assess the effectiveness of therapy or<br />

different aspects of rehabilitation activities. It could be an option to validate MVG-RIM2<br />

<strong>with</strong> other <strong>in</strong>struments currently tested <strong>in</strong> other countries even though MVG-RIM2 will<br />

at the earliest be implemented <strong>in</strong> Belgium <strong>in</strong> 2008.<br />

Organ<strong>is</strong>ation and f<strong>in</strong>anc<strong>in</strong>g of rehabilitation was analysed <strong>in</strong> five countries. All countries<br />

are struggl<strong>in</strong>g <strong>with</strong> these <strong>is</strong>sues and develop<strong>in</strong>g new solutions.<br />

In <strong>The</strong> Netherlands rehabilitation <strong>is</strong> organ<strong>is</strong>ed <strong>with</strong><strong>in</strong> 4 levels and patients need an<br />

<strong>in</strong>dication sett<strong>in</strong>g. <strong>The</strong>re <strong>is</strong> basic set of performance <strong>in</strong>dicators as well as a rehabilitation<br />

treatment framework, <strong>in</strong>tended as quality and accreditation <strong>in</strong>strument.<br />

In France rehabilitation <strong>is</strong> conceptually organ<strong>is</strong>ed around three levels of care and has a<br />

clear regional orientation. <strong>The</strong>re <strong>is</strong> no systematic model of <strong>in</strong>dication sett<strong>in</strong>g.<br />

Medical post-acute rehabilitation <strong>in</strong> Germany <strong>is</strong> ma<strong>in</strong>ly offered <strong>in</strong> special<strong>is</strong>ed<br />

rehabilitation cl<strong>in</strong>ics, although outpatient and part-time <strong>in</strong>patient care grows<br />

considerably. Approvals by the <strong>in</strong>surance companies for adm<strong>is</strong>sion to rehabilitation<br />

facilities are needed. Quality assurance programs <strong>in</strong>tend to impact on the allocation of<br />

patients as well as the f<strong>in</strong>anc<strong>in</strong>g of the rehabilitation services.<br />

In Sweden rehabilitation <strong>is</strong> organ<strong>is</strong>ed at county level, tak<strong>in</strong>g <strong>in</strong>to account the level of<br />

special<strong>is</strong>ation and the population’s needs. <strong>The</strong> rehabilitation has a clear orientation on<br />

home and ambulatory care, but a differentiation <strong>is</strong> made between at least two levels of<br />

facilities. <strong>The</strong> quality assessment <strong>is</strong> stimulated by both national health care agencies and
