aimed to communicate to the ma<strong>in</strong> collaborat<strong>in</strong>g <strong>in</strong>stitutions of the rehabilitation<br />
centres 144 .<br />
<strong>The</strong> aim of the quality approach <strong>is</strong> to develop and implement a set of 20 outcome<br />
measures <strong>in</strong> 2009. Some further work has to be done to further develop the system for<br />
specific patient groups. <strong>The</strong> development of the <strong>in</strong>dicators <strong>is</strong> a collaborative exerc<strong>is</strong>e of<br />
universities, rehabilitation physicians and research units, funded by VWS.<br />
In the framework of the development of <strong>in</strong>tegrated care for stroke patients (see <strong>in</strong>fra)<br />
some particular propositions are made for 11 performance <strong>in</strong>dicators for <strong>in</strong>tegrated<br />
stroke care. <strong>The</strong>se <strong>in</strong>dicators will be used <strong>in</strong> the “benchmark <strong>report</strong>s ketenzorg-CVA”.<br />
After an evaluation of pilot-<strong>in</strong>itiatives of <strong>in</strong>tegrated care <strong>in</strong> stroke (ed<strong>is</strong>se-study),<br />
<strong>in</strong>tegrated stroke care was <strong>in</strong>troduced <strong>in</strong> 23 dutch regions. For th<strong>is</strong> new <strong>in</strong>itiative, 19<br />
<strong>in</strong>dicators were used <strong>in</strong> order to assess the performance of the networks 151 . <strong>The</strong>se<br />
performance <strong>in</strong>dicators have yet no formal endorsement as a quality <strong>in</strong>strument.<br />
• <strong>The</strong> Dutch rehabilitation concept differentiates 4 levels of rehabilitation on<br />
a cont<strong>in</strong>uum: “general and simple” “general multi-d<strong>is</strong>cipl<strong>in</strong>ary”<br />
“special<strong>is</strong>ed and specific” and “top reference”.<br />
• Rehabilitation <strong>is</strong> organized <strong>in</strong> acute hospital sett<strong>in</strong>gs as well as <strong>in</strong> 24<br />
Rehabilitation Centres throughout the country and partly <strong>in</strong> nurs<strong>in</strong>g<br />
homes. Fourteen rehabilitation centres have the status of “top-reference”<br />
centre (l<strong>in</strong>k <strong>with</strong> universities).<br />
• Patients <strong>in</strong> rehabilitation <strong>in</strong>stitutions need an <strong>in</strong>dication sett<strong>in</strong>g.<br />
• <strong>The</strong> rehabilitation sector will be f<strong>in</strong>anced <strong>in</strong> the DBC model <strong>in</strong> 2008. Until<br />
then the f<strong>in</strong>anc<strong>in</strong>g <strong>is</strong> based on an activity based model.<br />
• “Function-oriented” f<strong>in</strong>anc<strong>in</strong>g for <strong>in</strong>tramural health care under AWBZ <strong>is</strong><br />
<strong>in</strong> place for <strong>in</strong>patient long term care-facilities. A further f<strong>in</strong>anc<strong>in</strong>g reform <strong>is</strong><br />
prepared <strong>in</strong>clud<strong>in</strong>g the “level of care” offered (calculated on the average<br />
number of hours of care and treatment for a certa<strong>in</strong> level of severity).<br />
• <strong>The</strong> quality approach <strong>in</strong> rehabilitation will have to fit the DBC f<strong>in</strong>anc<strong>in</strong>g<br />
and performance model. A “basic set” of performance <strong>in</strong>dicators <strong>is</strong> be<strong>in</strong>g<br />
developed and reg<strong>is</strong>tered. “Rehabilitation treatment frameworks” are<br />
<strong>in</strong>tended as quality and accreditation <strong>in</strong>struments. <strong>The</strong>y describe the set of<br />
m<strong>in</strong>imal conditions to be met when provid<strong>in</strong>g rehabilitation activities.<br />
8.3.5 Example: Stroke<br />
In recent years, a lot of “project based” attention has been paid to the organ<strong>is</strong>ation of<br />
facilities for people <strong>with</strong> stroke. Different pilot <strong>in</strong>itiatives have been launched to<br />
guarantee better coord<strong>in</strong>ated and <strong>in</strong>tegrated care between acute, post-acute, and home<br />
care arrangements (CVA-ketenzorg). <strong>The</strong> “Comm<strong>is</strong>sie CVA-Revalidatie”, a work<strong>in</strong>g<br />
group of the “Nederlandse Hartsticht<strong>in</strong>g” (www.hartsticht<strong>in</strong>g.nl), recommended <strong>in</strong> 2001<br />
“stroke services” (conceptualized as <strong>in</strong>tegration of rehabilitation activities) as the best<br />
way to take care of stroke patients, based on the op<strong>in</strong>ion of the experts of the work<strong>in</strong>g<br />
group. Apart from the network<strong>in</strong>g, they adv<strong>is</strong>ed to assure cont<strong>in</strong>uity of care by<br />
transmural patient notes and/or by a transmural nurse.<br />
<strong>The</strong> concept of stroke unit <strong>is</strong> used for a specific neurology department provid<strong>in</strong>g<br />
(sub)acute treatment <strong>with</strong><strong>in</strong> the hospital. On average people stay between 10-14 days.<br />
About 80% of the hospitals have such a stroke unit. ‘Special<strong>is</strong>ed rehabilitation stroke<br />
units’ are part of rehabilitation centres, aim<strong>in</strong>g at rehabilitation for <strong>in</strong>dependent liv<strong>in</strong>g.<br />
<strong>The</strong> duration of stay can be up to six months. A ‘nurs<strong>in</strong>g rehabilitation stroke unit’ <strong>is</strong> a<br />
special<strong>is</strong>ed department <strong>in</strong> nurs<strong>in</strong>g-facilities, most of the time rehabilitat<strong>in</strong>g people that<br />
are probably not return<strong>in</strong>g home. <strong>The</strong>re are only few units <strong>in</strong> each region. Some care<br />
<strong>in</strong>stitutions for the elderly have some specific units for elderly stroke patients. As the
