post-acute care quality assessment related to the potentially different character<strong>is</strong>tics <strong>in</strong><br />

patient populations served by different types of providers. Bryant et al conclude that it<br />

may not be possible to construct a s<strong>in</strong>gle dataset that by itself meets all needs <strong>in</strong> each<br />

sett<strong>in</strong>g. However, a s<strong>in</strong>gle core set of measures applicable to all sett<strong>in</strong>gs <strong>is</strong> considered to<br />

be useful for assess<strong>in</strong>g quality, facilitat<strong>in</strong>g transfers between providers, and m<strong>in</strong>imiz<strong>in</strong>g any<br />

data collection burden for healthcare professionals.<br />

4.3 THE BELGIAN NURSING MINIMUM DATA SETS<br />

4.3.1 Background<br />

<strong>The</strong> Belgian Nurs<strong>in</strong>g M<strong>in</strong>imum Data Set (BNMDS) <strong>is</strong> a reg<strong>is</strong>tration of nurs<strong>in</strong>g activities<br />

implemented nationally <strong>in</strong> 1988. Four times per year, dur<strong>in</strong>g a two-week period, patients<br />

<strong>in</strong> hospitals are scored on the level of nurs<strong>in</strong>g activities. <strong>The</strong> results of these scores are<br />

presented as so called “f<strong>in</strong>gerpr<strong>in</strong>ts” of hospital departments, projected on a national map.<br />

<strong>The</strong> reference for th<strong>is</strong> national compar<strong>is</strong>on <strong>is</strong> the “average” Belgian nurs<strong>in</strong>g unit.<br />

4.3.2 F<strong>in</strong>gerpr<strong>in</strong>ts of rehabilitation departments<br />

<strong>The</strong> f<strong>in</strong>gerpr<strong>in</strong>ts identify the nurs<strong>in</strong>g care on a nurs<strong>in</strong>g department organ<strong>is</strong>ed around 23<br />

activities (see Appendix to chapter 4) <strong>The</strong> f<strong>in</strong>gerpr<strong>in</strong>t <strong>is</strong> presented as horizontal<br />

h<strong>is</strong>togrammes for each of the 23 dimensions. Each variable <strong>is</strong> scored between -0,5 and<br />

+0,5. A score 0 implies that the department scores identical to the average Belgian nurs<strong>in</strong>g<br />

department. <strong>The</strong> calculations of the scores are based on a Ridit-analys<strong>is</strong>.<br />

• Example: <strong>The</strong> reference value 0 for the dimension “hygiëne”, <strong>is</strong> based on<br />

the observation that 35% of the patients <strong>in</strong> general hospitals <strong>is</strong> not<br />

gett<strong>in</strong>g help <strong>in</strong> hygienic care, 30% <strong>is</strong> receiv<strong>in</strong>g supportive help, 20% partial<br />

help, 15% full help. Each nurs<strong>in</strong>g unit <strong>is</strong> then compared to th<strong>is</strong> reference<br />

po<strong>in</strong>t. A higher percentage patients need<strong>in</strong>g a higher level of care, will<br />

lead to a positive score on the f<strong>in</strong>gerpr<strong>in</strong>t. A lower percentage of<br />

patients need<strong>in</strong>g more care will lead to a negative score<br />

rehabilitation department. <strong>The</strong>se average departments are characterized by a large (always<br />

compared to the average nurs<strong>in</strong>g unit) amount of basic nurs<strong>in</strong>g care (hygiene, mobility,<br />

excretion and nour<strong>is</strong>hment), a lot of day cloth<strong>in</strong>g tasks, comfort care (special mouth care<br />

and pressure ulcer care), and tra<strong>in</strong><strong>in</strong>g for <strong>in</strong>dependency (programme). <strong>The</strong> units have little<br />

technical nurs<strong>in</strong>g care. Figure 4.2 shows a similar f<strong>in</strong>gerpr<strong>in</strong>t of an average Belgian<br />

neurological rehabilitation department.
