7.5.2.1 Patient character<strong>is</strong>tics<br />
<strong>The</strong> patients <strong>in</strong>cluded <strong>in</strong> the pathways are patients who had an elective total hip<br />
replacement.<br />
Eight acute cl<strong>in</strong>ical pathways are deal<strong>in</strong>g <strong>with</strong> the immediate <strong>in</strong>patient care after surgery.<br />
One pathway (from the UK) describes an <strong>in</strong>patient rehabilitation service as well as an<br />
outpatient rehabilitation service. <strong>The</strong> Belgian pathway only considers the post-acute<br />
phase dur<strong>in</strong>g hospital<strong>is</strong>ation. It describes the care <strong>in</strong> a rehabilitation facility after<br />
d<strong>is</strong>charge from the orthopaedic ward.<br />
7.5.2.2 Delay after THR<br />
Six of the n<strong>in</strong>e acute cl<strong>in</strong>ical pathways mention the start of therapy on the first day after<br />
surgery. In three pathways rehabilitation starts on the day of surgery.<br />
One pathway describes rehabilitation <strong>in</strong> the post-acute phase after total hip<br />
replacement. No <strong>in</strong>formation <strong>is</strong> given on the delay after total hip replacement, it <strong>is</strong><br />
def<strong>in</strong>ed as start<strong>in</strong>g after d<strong>is</strong>charge from the acute care sett<strong>in</strong>g.<br />
7.5.2.3 Method of identification of patient needs<br />
Eight of the ten pathways describe a pre-adm<strong>is</strong>sion assessment of the patient. <strong>The</strong><br />
assessment tools mentioned <strong>in</strong> one pathway are nurs<strong>in</strong>g assessment, fall r<strong>is</strong>k<br />
assessment, thrombos<strong>is</strong> r<strong>is</strong>k assessment, wound heal<strong>in</strong>g r<strong>is</strong>k assessment, d<strong>is</strong>charge r<strong>is</strong>k<br />
assessment, Norton score and patient special needs assessment. Two other pathways<br />
use a questionnaire or a check l<strong>is</strong>t for pre-adm<strong>is</strong>sion assessment <strong>with</strong>out giv<strong>in</strong>g detailed<br />
<strong>in</strong>formation on the criteria. Seven of the n<strong>in</strong>e pathways describe the <strong>in</strong>volvement of a<br />
physical therap<strong>is</strong>t before adm<strong>is</strong>sion. Five out of the eight pathways have an occupational<br />
assessment as well. One pathway even describes the possibility of a pre-adm<strong>is</strong>sion home<br />
v<strong>is</strong>it.<br />
In all pre-adm<strong>is</strong>sion assessments the surgeon and a nurse are <strong>in</strong>volved.<br />
7.5.2.4 Conditions to start rehabilitation<br />
Total hip replacement <strong>is</strong> the only <strong>in</strong>clusion criterion. No other <strong>in</strong>- or exclusion criteria<br />
are mentioned.<br />
7.5.2.5 Duration of rehabilitation<br />
<strong>The</strong> length of stay <strong>in</strong> the acute care facility (= orthopaedic ward) ranges from 3 to 11<br />
days. <strong>The</strong> pathway <strong>with</strong> a length of stay of 3 days conta<strong>in</strong>s an outpatient rehabilitation<br />
service which <strong>in</strong>cludes a home care program <strong>with</strong> multid<strong>is</strong>cipl<strong>in</strong>ary rehabilitation. Th<strong>is</strong><br />
program lasts 7 days.<br />
<strong>The</strong> end of the acute phase <strong>is</strong> d<strong>is</strong>charge from the acute care facility.<br />
At d<strong>is</strong>charge, all pathways mention physical therapy as an outpatient rehabilitation<br />
service. In the pathways from the UK home-based occupational therapy <strong>is</strong> also supplied<br />
if necessary. Different d<strong>is</strong>charge dest<strong>in</strong>ations are possible. <strong>The</strong> pathways do not mention<br />
criteria for d<strong>is</strong>charge to a rehabilitation centre.<br />
One post-acute pathways starts after d<strong>is</strong>charge from the orthopaedic ward. <strong>The</strong> length<br />
of stay <strong>in</strong> the rehabilitation centre varies between 23 and 40 days.<br />
7.5.2.6 Involved professionals<br />
Concern<strong>in</strong>g the <strong>in</strong>volved professionals, we conclude that all pathways conta<strong>in</strong> a physical<br />
therap<strong>is</strong>t. Eight of the ten pathways <strong>in</strong>clude occupational therapy. Other therap<strong>is</strong>ts<br />
mentioned are a social worker, a recreational therap<strong>is</strong>t and a speech therap<strong>is</strong>t.
