8.7.6 Example: Total Hip Replacement<br />

Prior to the <strong>in</strong>troduction of the pre-paid system, patients undergo<strong>in</strong>g total hip<br />

replacement rema<strong>in</strong>ed <strong>in</strong> the acute hospital until they could be d<strong>is</strong>charged home. Due<br />

to the <strong>in</strong>creas<strong>in</strong>g f<strong>in</strong>ancial pressure on acute sett<strong>in</strong>gs, lead<strong>in</strong>g to a reduction of length of<br />

stay, referral to <strong>in</strong>patient rehabilitation services <strong>in</strong>creased significantly as functional<br />

recovery was not establ<strong>is</strong>hed <strong>in</strong> the acute hospital. 225 , 226 Rehabilitation <strong>is</strong> offered <strong>in</strong><br />

various facilities: <strong>in</strong>patient rehabilitation facilities (IRFs), Skilled Nurs<strong>in</strong>g Facilities (SNFs)<br />

or long-term care hospitals (LCTH).<br />

Currently, referrals to post-acute facilities are not regulated and there are no criteria<br />

implemented def<strong>in</strong><strong>in</strong>g patients eligible for particular services. Dec<strong>is</strong>ion-mak<strong>in</strong>g on<br />

d<strong>is</strong>charge d<strong>is</strong>position <strong>is</strong> made by the physician <strong>in</strong> the acute hospital. In most cases, a<br />

d<strong>is</strong>charge planner will be <strong>in</strong>volved assur<strong>in</strong>g a smooth transition <strong>in</strong>to post-acute care<br />

sett<strong>in</strong>g. Percentages of patients be<strong>in</strong>g d<strong>is</strong>charged to one of the post-acute services vary<br />

between 33% 227 and 58% 228 , ma<strong>in</strong>ly depend<strong>in</strong>g on the age of the studied group.<br />

However, the specific dest<strong>in</strong>ation <strong>is</strong> more <strong>in</strong>fluenced by non-cl<strong>in</strong>ical factors. In a recent<br />

study by Bunt<strong>in</strong> et al. 207 availability of the post-acute facility as well as the network of<br />

the acute hospital were better predictors for d<strong>is</strong>charge dest<strong>in</strong>ation than any cl<strong>in</strong>ical<br />

<strong>in</strong>dicator.<br />

As service delivery varies largely between these post-acute facilities, it <strong>is</strong> expected that<br />

outcome will vary considerably. Recent studies showed superior outcomes for patients<br />

admitted to IRFs <strong>in</strong> compar<strong>is</strong>on to SNF-patients. 229 , 230 <strong>The</strong> hypothes<strong>is</strong> <strong>is</strong> made that th<strong>is</strong><br />

<strong>is</strong> due to the more <strong>in</strong>tensive rehabilitation programs <strong>in</strong> the IRFs. <strong>The</strong> average length of<br />

stay varies between 10 days and 13 days ma<strong>in</strong>ly dependent on the need for rev<strong>is</strong>ion<br />

surgery and age.<br />

8.8 INTERNATIONAL COMPARISON<br />

8.8.1 A basic schematic compar<strong>is</strong>on of the different countries<br />

Based on the <strong>in</strong>formation gathered we tried to schematically synthes<strong>is</strong>e the organ<strong>is</strong>ation<br />

of musculoskeletal and neurological rehabilitation for the five different countries. As can<br />

be expected, we do not claim that these schemes cover al details and nuances. <strong>The</strong>y are<br />

made as a tool for a quick compar<strong>is</strong>on, and as a stepp<strong>in</strong>g stone for the<br />

recommendations <strong>in</strong> the f<strong>in</strong>al chapter.
