CONDITIONS REQUIRING PERMITS, AUTHORIZATIONS OR FILINGS:<br />
Owners and operators of facilities that accept used oil from any other generators of used oil,<br />
other than individual households, must register the facility with ADEQ. ADEQ has an<br />
application form for this purpose. Used oil collection centers may also accept used oil from<br />
household do-it-yourselfers. For clarification regarding the applicability of this requirement,<br />
contact ADEQ at the above address or phone number.<br />
Owners/operators of used oil collection centers must provide information regarding:<br />
A used oil collection center, as defined in 40 CFD, Part 279, shall register with the department<br />
by obtaining an identification number from the department. A request for an identification<br />
number shall include:<br />
(a) The company name.<br />
(b) The name of the owner of the company.<br />
(c) The mailing address and telephone number of the company.<br />
(d) The location of the collection center.<br />
(e) A description of the type of used oil activity at the company.<br />
