Used Oil Burner - Annual Report<br />
LEGAL AUTHORITY:<br />
A.R.S. § 49-802.C.4<br />
CONDITIONS REQUIRING PERMITS, AUTHORIZATIONS OR FILINGS:<br />
Each used oil burner is required to submit a written annual report of its used oil activities to<br />
ADEQ. Each annual report shall provide information about the company's used oil activities<br />
for the preceding calendar year.<br />
ADEQ has developed a “standardized” reporting form that can be can be downloaded at the<br />
web link listed above; however, if the used oil burner wishes to use a different form for the<br />
reporting requirement, each written report must provide the following information for the<br />
preceding calendar quarter.<br />
The report shall be submitted to the department by February 1 for the previous calendar year<br />
and shall contain the following information:<br />
(a) The name, address and telephone number of the person reporting.<br />
(b) The name, address and telephone number of the burner facility.<br />
(c) The United States environmental protection agency identification number of the<br />
burner facility.<br />
(d) The total volume of on-specification used oil burned.<br />
(e) The period being reported.<br />
(f) The total volume of self-generated used oil burned on site.<br />
(g) The total volume of used oil fuel burned.<br />
