Used Oil Handlers - EPA Identification Number<br />
Requirements for Transporters, Processors/Re-Refiners, Marketers and Burners<br />
EPA Identification Number.<br />
The EPA Form 8700-12: RCRA Subtitle C Site Identification Form (Notification of<br />
Regulated Waste Activity Instructions and Form Booklet) can be downloaded at the web link<br />
listed below. Once completed, the document (with an original signature), should be mailed<br />
to the address listed above. Once processed, ADEQ will send the facility confirmation of<br />
registration.<br />
Used oil transportation, processing, burning and marketing activities should not commence<br />
until receipt of the ADEQ confirmation of the EPA ID Number assignment.<br />

CONDITIONS REQUIRING PERMITS, AUTHORIZATIONS OR FILINGS:<br />
An EPA identification number is required for transporters/transfer facilities, processors/rerefiners,<br />
marketers and burners of used oil, prior to activity.<br />
EXEMPTIONS:<br />
A used oil burner of "on-specification" used oil fuel is not required to obtain an EPA ID<br />
number; however, the burner may want to consider obtaining an EPA ID number in the event<br />
he would accidentally burn "off-specification" used oil fuel. (Anyone who burns "offspecification"<br />
used oil fuel without an EPA ID number, or the appropriate air quality permit,<br />
is subject to enforcement action by ADEQ.)<br />
