2. Decisions must be made by the General Education Homebound Committee<br />
a. Consideration of physician’s information (In making these decisions the GEH<br />
committee must consider the physician’s information. However, the physician’s<br />
information is not the sole determining factor in the committee’s decision making<br />
process).<br />
b. If the decision is to provide general education homebound services, then the<br />
committee must determine the type and amount of instruction to be provided to the<br />
student and the subjects that will be taught on Homebound. Due to limited direct<br />
instructional time from teachers, these types of classes may be deemed<br />
inappropriate for Homebound Services by the committee: some electives, labs, AP<br />
level classes, etc.<br />
3. Requirements to Consider<br />
a. Using the General Education Homebound Meeting Form (GH # 2) which<br />
documents GEH committee decisions regarding whether or not a student is to be<br />
served through GEH.<br />
b. The GEH committee must review the Medical Evaluation Report from the<br />
physician (GEH # 1) stating the student has a medical condition which requires the<br />
student to be confined at home/hospital bedside for a minimum of four (4)<br />
consecutive weeks.<br />
c. The GEH committee must review data to help determine the number of hours per<br />
week and the classes to be taught during Homebound Services such as: the health<br />
of the student---is the student capable of maintaining the rigor of the subject matter<br />
and course work as is or will it have to be significantly modified? What are the<br />
critical subjects that Homebound could provide that can maintain the integrity of<br />
the subject/course? Due to the severity of the illness/injury, in many cases four<br />
hours a week (the minimum) may be the limit of direct instructional time a student<br />
can attend without fatigue or detriment to the students health may occur; the length<br />
of time the student will be on homebound---will it be long term or only for 4-6<br />
weeks?; the student’s attendance---is the student able to attend school for even 1 or<br />
2 periods a day to access classes and would this be better for the student than<br />
Homebound Services?; grades---what are the current and past grades of the<br />
student? If the student is not doing well, would it be best to drop the course until<br />
the student can return to school?; is there a better way to obtain instruction in the<br />
course--could the work and tests generated by the student’s general education<br />
teachers be transported another way that would maintain the expectations of the<br />
course? Is the general education teacher willing to support the student if the work<br />
is transported back and forth? Could email for assignments be utilized? Could<br />
PLATO be utilized (if the home has internet access?)<br />
d. The GEH committee must document on the form (GEH # 2) the committee’s<br />
decision regarding the type(s) and amount of instruction to be provided to the<br />
student. This must include the designated amount of time per week that instruction<br />
will be provided and for what subjects.<br />
e. Documentation of the day(s) homebound instruction started and stopped.<br />
Meeting to Return the Student to <strong>School</strong><br />
1. When the physician provides a medical release date to return to school, the GEH Committee<br />
meets and completes the GEH form # 2 to dismiss the student from Homebound Services.<br />
The parent could participate by phone if they wish.<br />
2. File the General Education Homebound Summary Form # 3 and the General Education<br />
Homebound <strong>Teacher</strong> Contact Log From # 5 along with the other General Education<br />
Homebound Forms from the first meeting in the student’s cumulative folder.<br />
