Z. GENERAL EDUCATION HOMEBOUND PROCEDURES (GEH)<br />
General Education Homebound Program Criteria (GEH)<br />
1. Student is expected to be confined at home or hospital bedside for a minimum of four (4)<br />
consecutive weeks.<br />
2. Student is expected to be confined at home or hospital bedside within <strong>Aldine</strong> ISD<br />
boundaries for medical reasons only.<br />
3. The medical condition is documented by a physician licensed to practice in the United<br />
States.<br />
4. Students must be served by a certified general education teacher.<br />
Steps for General Education Homebound Consideration<br />
1. Parent lets nurse know that the student is ill/had surgery/severe injury etc.<br />
2. Nurse explains criteria for placing a student on homebound (Criteria-doctor expects the<br />
student to be out of school for a minimum of four consecutive weeks from the date the<br />
doctor completes the Medical Evaluation)<br />
3. Nurse gives the parent a copy of the Medical Evaluation Report (MER) Form GH #1 has<br />
parent sign consent for school to speak to doctor, gives parent homebound parent<br />
letter, and has parent sign Receipt of Homebound Information (GE-0009).<br />
4. Nurse notifies administrator of possible homebound placement.<br />
5. IF STUDENT DOES NOT WARRANT HOMEBOUND:<br />
a. Nurse notifies Campus Administrator<br />
b. Campus Administrator notifies Parent<br />
c. If parent disagrees, Nurse notifies Program Director of Health Services as an<br />
independent mediator<br />
6. IF STUDENT WARRANTS HOMEBOUND:<br />
a. Nurse notifies the campus EIT coordinator to schedule a GEH Meeting<br />
b. GEH Meeting is held, nurse notifies Homebound immediately.<br />
7. EIT Coordinator makes a copy of the forms: GEH # 1, GEH # 2. EIT Coordinator collects<br />
the student’s class schedule, a copy of the student’s transcript, a copy of the student’s most<br />
recent report card and a copy of the student’s blue card. Send the copy of all of these<br />
documents to Homebound Services in a folder within 2 days of the General Education<br />
Homebound Meeting. Keep the originals on the home campus.<br />
General Education Homebound Meeting<br />
1. Role<br />
a. Review and consider the necessity of providing instruction to a general education<br />
student at home/hospital bedside.<br />
2. Decisions must be made by the General Education Homebound Committee<br />
a. Consideration of physician’s information (In making these decisions the GEH<br />
committee must consider the physician’s information. However, the physician’s<br />
information is not the sole determining factor in the committee’s decision making<br />
process).<br />
b. If the decision is to provide general education homebound services, then the<br />
committee must determine the type and amount of instruction to be provided to the<br />
student and the subjects that will be taught on Homebound. Due to limited direct<br />
instructional time from teachers, these types of classes may be deemed<br />
inappropriate for Homebound Services by the committee: some electives, labs, AP<br />
level classes, etc.<br />
