14. If the employee is unable to<br />

provide at least 45 mL, the<br />

collector will discard the<br />

specimen and follow the<br />

insufficient volume procedures<br />

described later in this chapter.<br />

15. If during the collection process,<br />

the collector notices any conduct<br />

that clearly indicates an attempt<br />

to substitute or adulterate a<br />

specimen, the collection process<br />

must be completed and then the<br />

collector must immediately begin<br />

a new collection under direct<br />

observation as described later in<br />

this chapter. In addition, the<br />

collection site technician must<br />

visually examine the specimen<br />

for any unusual color, sediment,<br />

or other signs of tampering, and<br />

note the results on the custody<br />

and control form. If it is<br />

apparent that the employee has<br />

tampered with the specimen, the<br />

collector must immediately begin<br />

a new collection under direct<br />

observation.<br />

16. If the employee refuses to<br />

cooperate with the collection<br />

process including conducting a<br />

new test under direct<br />

observation, the collector will<br />

immediately inform the DER and<br />

document the noncooperation on<br />

the <strong>Federal</strong> Drug Testing<br />

Custody and Control form.<br />

17. In the presence of the donor, the<br />

collector will pour the urine into<br />

two specimen bottles. Thirty<br />

(30) mL will be poured into the<br />

primary specimen or “A” bottle<br />

and the cap will be placed and<br />

secured on the bottle. Then the<br />

collector will pour at least 15 mL<br />

into the split specimen or “B”<br />

bottle and the cap will be placed<br />

and secured on the bottle.<br />

18. The collector will seal and label<br />

the specimen bottles in the<br />

presence of the donor. The<br />

preprinted label(s) with the same<br />

specimen identification number<br />

as the custody and control form<br />

will be removed from the forms<br />

and attached to the specimen<br />

bottles. The collector will ensure<br />

that the “A” label is placed on<br />

the primary specimen and the<br />

“B” label is placed on the split<br />

specimen. The collector will<br />

then write the date on the seals<br />

and then request the employee to<br />

initial the labels verifying that<br />

the specimen is his/hers. The<br />

employee must be present to<br />

observe the sealing of the bottles.<br />

The employee must initial the<br />

labels after they have been<br />

attached to the bottle, not before.<br />

19. The remaining steps of the<br />

<strong>Federal</strong> Drug Testing Custody<br />

and Control form (CCF) must be<br />

completed. The collector will<br />

direct the employee to read, sign,<br />

and date the certification<br />

statement, and provide date of<br />

birth, printed name, and day and<br />
