Have the collection site staff demonstrated proficiency?<br />

Have the collection site staff met the refresher training requirements?<br />

Has error correction training been provided?<br />

Are all Chain of Custody procedures followed?<br />

Supplies<br />

Are the following supplies, equipment, and documents used at each<br />

collection site?<br />

⎯ Single-use collection cups: The plastic cups must be<br />

individually and securely wrapped. The cups are unwrapped in<br />

the presence of the employee at the time of the testing.<br />

⎯ Single-use specimen bottles: The bottles must be constructed of<br />

plastic with a screw-on or snap-on cap that prevents seepage.<br />

The bottles must be individually and securely wrapped and<br />

opened in the presence of the employee at the time of the<br />

collection. Each bottle must hold at least 35 ml of urine.<br />

⎯ Single-use temperature measurement device: The device<br />

must be capable of measuring temperatures within the range of<br />

90.0 to 100.0 F.<br />

⎯ <strong>Federal</strong> Drug Testing Custody and Control Form: This form<br />

documents the exchanges of the specimen from the time of<br />

production by the donor until the test is completed. Only US<br />

DOT forms can be used for collections made under FTA<br />

authority (revised forms must be used after 8/1/01).<br />

⎯ Tamper-evident sealing system: Pre-printed seals should be<br />

provided to ensure that the specimen bottle has not been opened.<br />

The seals must have an identifying number that is identical to the<br />

number appearing on the <strong>Federal</strong> Drug Testing Custody and<br />

Control Form.<br />

⎯ Leak-resistant Plastic Bag: The plastic bag must have two<br />

sealable compartments or pouches that have a tamper-evident<br />

seal. The specimens are placed in one pouch, the CCF is placed<br />

in the other.<br />

⎯ Shipping containers: The shipping containers must be designed<br />

to adequately protect the specimen bottles from shipment<br />

damage.<br />
