⎯ Turn off, tape, or prevent the use of other sources of water (e.g.,<br />

sink or shower) that are located in the privacy enclosure where<br />

urination occurs?<br />

⎯ Remove all potential adulterants?<br />

⎯ Secure areas suitable for concealing contaminants such as trash<br />

receptacles, paper towel holders, etc.?<br />

Does the collection site have a procedure in place for notifying the<br />

employer if the employee does not report for the test in the designated<br />

time frame?<br />

Do you have a procedure to notify the collection site of the identity and<br />

contact information of the Designated Employer Representative (DER)?<br />

Does the collection site use the correct US DOT Chain of Custody and<br />

Control forms for DOT/FTA tests (and only DOT tests)?<br />

Is the specimen and CCF under the control of the collector throughout<br />

the collection process? Is the collector the only person that handles the<br />

specimen before it is sealed?<br />

Are “limited access” signs posted in areas of public access?<br />

Does the collection site restrict access to specimens and specimen<br />

collection materials?<br />

Are collection sites available to perform collections during all days and<br />

hours that the transit system performs safety-sensitive job duties?<br />

Do collectors recheck the privacy enclosure following the collection<br />

process?<br />

Has the collection site staff received the following required initial<br />

training?<br />

⎯ Basic information;<br />

⎯ Qualification training; and<br />

⎯ Trained to prepare the collection site, collect specimens, examine<br />

specimens for tampering or adulteration, observe collections,<br />

split the specimens, and properly label and preserve the chain of<br />

custody of specimens?<br />
