the CCF. The signature attests to<br />

the authenticity of the specimen<br />

and information provided and the<br />

integrity of the collection<br />

process. Refusal to sign the form<br />

does not constitute a refusal to<br />

test, but should be noted by the<br />

collector in the remarks section<br />

of the form.<br />

The employee is not to put any<br />

additional personal information<br />

on the CCF. The collector must<br />

instruct the employee not to list<br />

any medications that he or she is<br />

currently taking on the CCF.<br />

The employee may make notes<br />

of medications on the back of the<br />

employee copy (Copy 5) of the<br />

CCF for his or her own personal<br />

use, but they must not be<br />

conveyed to anyone else in the<br />

drug testing process.<br />

20. The collector completes the<br />

collector’s portion of the CCF by<br />

printing his/her name, date and<br />

time of the collection, entering<br />

the name of the courier/delivery<br />

service and signing the<br />

certification statement. The CCF<br />

is checked to make sure it is<br />

complete and legible and the<br />

employee is provided Copy 5 of<br />

the form.<br />

21. Both the primary specimen and<br />

the split specimen must be sealed<br />

along with Copy 1 of the CCF in<br />

the appropriate pouches of the<br />

plastic bag. The employee will<br />

be notified that he/she can leave<br />

and the plastic bag will be<br />

readied for shipping to the<br />

laboratory (e.g., using shipping<br />

containers and following<br />

procedures defined by the courier<br />

service). The specimen should be<br />

placed in secure storage until<br />

picked up by the courier/delivery<br />

service.<br />

22. Copy 2 of the CCF will be sent to<br />

the MRO and Copy 4 of the CCF<br />

will be sent to the DER within 24<br />

hours, or the next business day.<br />

The specimen must be shipped to<br />

the laboratory as soon as<br />

possible, but no later than the 24<br />

hours or the next business day.<br />

Any urine specimen remaining after the<br />

specimen bottles have been filled and sealed<br />

should be discarded. If the agency tests for<br />

drugs other than those specified by the FTA<br />

regulation, a completely separate urine<br />

collection with its own non-federal custody<br />

and control form is required.<br />

Split Sample. As described, the DOT<br />

drug testing process requires that the urine<br />

specimen be split and poured into two<br />

specimen bottles. The split specimen<br />

process provides the employee with the<br />

option of having an analysis of the split<br />

sample performed at a separate DHHS<br />

laboratory should the primary specimen test<br />

result come into question. The employee<br />

has 72 hours after being informed by the<br />

MRO of a non-negative test result (i.e.,<br />

positive, adulterated, substitute) to request a<br />

test of the split sample.<br />

Insufficient Volume (§40.193,<br />

§40.195). If an employee is unable to<br />

provide 45 mL of urine, the collector will<br />

discard the specimen and instruct the<br />

employee that he/she has up to 3 hours to<br />

provide another specimen with sufficient<br />

volume. The 3-hour time period begins<br />

when the insufficient quantity or empty cup<br />

is presented to the collector. During the 3hour<br />

period, the employee may consume up<br />

to 40 ounces of fluid distributed reasonably<br />

throughout the period. The employee’s<br />
