by service type. Follow<strong>in</strong>g this alphabetic list is a numeric list <strong>of</strong> program def<strong>in</strong>itions and<br />

the correspond<strong>in</strong>g codes.<br />

Service providers who operate more than one certified chemical dependence site must report<br />

each site separately (<strong>in</strong> accordance with the approved budget) by <strong>in</strong>dex<strong>in</strong>g the appropriate<br />

program code as <strong>in</strong>dicated <strong>in</strong> the examples below:<br />

Example: A service provider operat<strong>in</strong>g an outpatient medically supervised chemical<br />

dependence cl<strong>in</strong>ic with three certified sites would report program codes 3520-00,<br />

3520-01, 3520-02.<br />
