Select the position title and code that reflects functions performed by the <strong>in</strong>dividual(s) and enter<br />
the appropriate title and code number on Schedule Consolidated Fiscal Report (<strong>CFR</strong>)-4 and, if<br />
applicable, Schedule <strong>CFR</strong>-4A.<br />
Note: Certa<strong>in</strong> position titles are unique to <strong>in</strong>dividual <strong>New</strong> <strong>York</strong> <strong>State</strong> agencies. Be<br />
certa<strong>in</strong> that the title used is acceptable for the <strong>New</strong> <strong>York</strong> <strong>State</strong> agency that provides your<br />
fund<strong>in</strong>g. <strong>Office</strong> <strong>of</strong> <strong>Mental</strong> <strong>Health</strong> (OMH) service providers should note that certa<strong>in</strong><br />
position title codes are only acceptable for certa<strong>in</strong> types <strong>of</strong> OMH programs.<br />
• Agency adm<strong>in</strong>istration staff must be assigned position title codes from 600<br />
through 699.<br />
• Local Governmental Unit (LGU program code 0890) staff must be assigned<br />
position title codes from 700 through 799.<br />
• Program adm<strong>in</strong>istration staff must be assigned position title codes from 500<br />
through 599.<br />
• Program/site staff must be assigned position title codes from 100 through 499.<br />
