4040 - Teach<strong>in</strong>g Family Home<br />

(Licensed Program)<br />

The Teach<strong>in</strong>g Family Homes are designed to provide <strong>in</strong>dividualized care to children and youth<br />

with serious emotional disturbances <strong>in</strong> a family-like, community-based environment. Specially<br />

tra<strong>in</strong>ed parents live and work with four children and youth with serious emotional disturbances <strong>in</strong><br />

a home-like sett<strong>in</strong>g. The teach<strong>in</strong>g parents are responsible for the social education <strong>of</strong> the<br />

children and the implementation <strong>of</strong> a service plan developed <strong>in</strong> conjunction with the family and<br />

cl<strong>in</strong>ical service provider. The focus <strong>in</strong> on teach<strong>in</strong>g the youth to live successfully <strong>in</strong> a family,<br />

attend school, and live productively <strong>in</strong> the community.<br />

This is a type <strong>of</strong> Licensed Hous<strong>in</strong>g/Community Residential program for children and<br />

adolescents as def<strong>in</strong>ed <strong>in</strong> 14NYCRR594.<br />

Units <strong>of</strong> Service: Count each resident day.<br />

4340 - Ongo<strong>in</strong>g Integrated Supported Employment Services<br />

(Non-Licensed Program)<br />

These funds are <strong>in</strong>tended for ongo<strong>in</strong>g job ma<strong>in</strong>tenance services <strong>in</strong>clud<strong>in</strong>g job coach<strong>in</strong>g,<br />

employer consultation, and other relevant supports needed to assist an <strong>in</strong>dividual <strong>in</strong> ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>g<br />

a job placement. These services are <strong>in</strong>tended to complement VESID time-limited supported<br />

employment services.<br />

5070 - Supported S<strong>in</strong>gle Room Occupancy (SP-SRO)<br />

(Non-Licensed Program)<br />

A s<strong>in</strong>gle-room occupancy residence which provides long term or permanent hous<strong>in</strong>g <strong>in</strong> a sett<strong>in</strong>g<br />

where residents can access the support services they require to live successfully <strong>in</strong> the<br />

community. Front desk coverage is provided 24 hours per day. <strong>Mental</strong> health service supports<br />

are provided either by SP-SRO staff or nonresidential service providers <strong>in</strong> accordance with a<br />

service plan developed jo<strong>in</strong>tly by the provider and resident.<br />

5340 - Supported Education<br />

(Non-Licensed Program)<br />

The objective <strong>of</strong> this program is to provide mental health and rehabilitation services to<br />

<strong>in</strong>dividuals with a serious mental illness to assist them to develop and achieve academic goals
