Sensitivity analyses<br />
When sufficient data were available we planned to explore the influence<br />
of some study characteristics by means of sensitivity analyses.<br />
We considered the following study characteristics:<br />
• concealment of allocation;<br />
• blinding;<br />
• extent of withdrawals and dropouts.<br />

From the initial searches per<strong>for</strong>med in 2004 and 2009 we identified<br />
215 potentially relevant citations. Of these, 19 were systematic<br />
and narrative reviews, which were further screened <strong>for</strong> additional<br />
trials (Ada 2006; Ada 2007; Andersen 2001; Barreca 2003; Eng<br />
2004; Ernst 1990; Giuliani 1995; Hiraoka 2001; Manning 2003;<br />
Meek 2003; Morris 2004; Moseley 2005; Pang 2006a; Ramas<br />
2007; Urton 2007; Van de Port 2007; Van der Lee 2001; Van<br />
Peppen 2004; Wagenaar 1991); 96 studies failed to meet the inclusion<br />
criteria; 58 potentially relevant studies remained unclassified<br />
because they were recently published and required either additional<br />
in<strong>for</strong>mation or translation into English in order to apply<br />
the inclusion criteria; 19 trials were still ongoing; 23 trials met the<br />
inclusion criteria.<br />
We updated the previous electronic search strategies and other relevant<br />
searches (<strong>for</strong> example handsearching, screening of conference<br />
proceedings and relevant websites) in 2010. We also checked<br />
all the references in both the studies awaiting classification and<br />
ongoing studies sections of the previous version of this review.<br />
We screened a total of 9481 citations. We identified 11 additional<br />
completed trials (Aidar 2007; Bale 2008; Cooke 2010; Donaldson<br />
2009; Flansbjer 2008; Langhammer 2007; Lennon 2008; Moore<br />
2010; Mudge 2009; Sims 2009; Smith 2008), two additional publications<br />
of an already included study (Katz-Leurer 2003), two<br />
papers reporting secondary analyses of an already included study<br />
(Duncan 2003), and seven ongoing trials. Five of the 11 new trials<br />
were previously in the waiting assessment section. Six of the seven<br />
ongoing trials were new and one was a published protocol of a<br />
study already included in Ongoing studies. We added a further 29<br />
studies to the table Characteristics of excluded studies. The most<br />
common reasons <strong>for</strong> exclusion were: a controlled trial in which the<br />
intervention did not meet the criteria <strong>for</strong> <strong>fitness</strong> <strong>training</strong> or did<br />
not include a suitable comparison, or a confounding of <strong>training</strong><br />
with another active physical intervention. We also excluded two<br />
trials that were previously included: Dean 2000 because the control<br />
group contained a degree of physical activity, and Pohl 2007<br />
because an assistive device was used as an adjunct to gait <strong>training</strong>.<br />
We revised the list of the excluded studies and we deleted studies<br />
that did not meet the pre-specified inclusion criteria <strong>for</strong> the type<br />
of study design or publication (that is not an RCT, abstract publication).<br />
This was done with the purpose of making the list of<br />
excluded studies more efficient and manageable.<br />
we included 32 trials in total. Two trials are dissertations (Cuviello-<br />
Palmer 1988; James 2002) and 14 trials have secondary publications<br />
(Cooke 2010; da Cunha 2002; Donaldson 2009; Duncan<br />
2003; Eich 2004; Flansbjer 2008; Katz-Leurer 2003; Langhammer<br />
2007; Mead 2007; Salbach 2004; Sims 2009; Richards 1993;<br />
Teixeira 1999; Winstein 2004). There were 121 excluded studies<br />
and 16 ongoing studies.<br />
Participants<br />
A total of 1414 <strong>stroke</strong> survivors (range 13 to 100 individuals, mean<br />
44.5, median 42) were randomised to physical <strong>fitness</strong> or control<br />
interventions in the 32 included clinical trials. The mean age of the<br />
<strong>patients</strong> was approximately 64 years. The mean time since onset<br />
of symptoms ranged from 8.8 days in trials assessing participants<br />
be<strong>for</strong>e discharge from hospital (Richards 1993) to 7.7 years in trials<br />
assessing participants after hospital discharge (Teixeira 1999).<br />
One trial (Richards 1993) recruited non-ambulatory <strong>stroke</strong> survivors,<br />
three trials (Bateman 2001; Cooke 2010; Lennon 2008)<br />
recruited both ambulatory and non-ambulatory participants, two<br />
trials (Donaldson 2009; Winstein 2004) did not report this in<strong>for</strong>mation,<br />
and all the remaining trials recruited ambulatory <strong>stroke</strong><br />
survivors.<br />
Participants were assessed at the end of the <strong>training</strong> period (end of<br />
intervention), or at any other defined point either within the trial<br />
duration or after completion of the <strong>training</strong> programme, or both<br />
(scheduled end of follow-up).<br />
