con<strong>di</strong>tions of the sample. 40 urine samples have been collected from<br />
workers exposed to benzene, both smokers and not smokers, and for<br />
each sample the percentage of SPMA measurable at pH 2 and<br />
without pH correction (free SPMA) has been calculated with respect<br />
to the SPMA measured after quantitative hydrolysis, with the<br />
objectives to determine if a correct assessment of the exposure<br />
requires the determination of total SPMA and which concentration<br />
value could correspond to the BEI of 25 µg/g of creatinine<br />
established by the ACGIH. An aliquot of the urine samples has been<br />
treated with 9M H 2 SO 4 , a second one is brought to pH 2 and a third<br />
one is analyzed as it is. All samples are analyzed by HPLC/MS/MS<br />
in negative ions/MRM mode, and quantitative analysis is performed<br />
using the internal standard method. The percentage found in<br />
samples treated at pH 2 is on average 45% of the total SPMA for<br />
smokers and 60% for non smokers, while the free SPMA varies from<br />
1% to 66% due to the urine pH variability and to the lower<br />
concentrations detected. The determination of total SPMA allows the<br />
standar<strong>di</strong>zation of the preanalytical factors and the dosage with<br />
analytical methods less sensitive than HPLC/MS/MS.<br />

INTRODUZIONE<br />
L’acido S-fenilmercapturico (SPMA) è uno degli in<strong>di</strong>catori<br />
biologici proposti dall’ACGIH per la valutazione<br />
dell’esposizione professionale a benzene, con un valore<br />
limite <strong>di</strong> 25 µg/g <strong>di</strong> creatinina nell’urina <strong>di</strong> fine turno<br />
(1). Stu<strong>di</strong> effettuati su soggetti esposti a benzene lamentano<br />
una mancanza <strong>di</strong> correlazione fra la concentrazione<br />
ambientale <strong>di</strong> benzene e i risultati del monitoraggio<br />
biologico (2). Una possibile causa è l’esistenza nelle<br />
urine <strong>di</strong> N-acetyl-S (1,2-<strong>di</strong>hydro-2-hydroxyphenyl)-Lcysteine,<br />
un precursore dell’SPMA (pre-SPMA) che può<br />
essere trasformato in quest’ultimo per idrolisi acida<br />
(3,4). La trasformazione quantitativa del pre-SPMA in<br />
SPMA avviene per trattamento con H 2 SO 4 9M, mentre a<br />
pH superiori è parziale.<br />
Nei meto<strong>di</strong> analitici utilizzati per la determinazione<br />
dell’SPMA urinario il pH non è considerato un fattore<br />
critico. In alcuni il campione <strong>di</strong> urina viene aci<strong>di</strong>ficato<br />
intorno a pH 2 (5), mentre in altri viene utilizzato acido<br />
acetico al 10% nel corso della SPE (6). Quin<strong>di</strong> la quantità<br />
<strong>di</strong> SPMA misurato che <strong>di</strong>pende dal grado <strong>di</strong> idrolisi,<br />
varia in funzione del pH iniziale e delle<br />
modalità <strong>di</strong> conservazione e trattamento<br />
del campione <strong>di</strong> urina. Gli obiettivi del presente<br />
lavoro sono: analizzare campioni <strong>di</strong><br />
soggetti esposti a <strong>di</strong>verse concentrazioni <strong>di</strong><br />
benzene per determinare quale sia il rapporto<br />
fra la quantità <strong>di</strong> SPMA totale (ottenuto<br />
per idrolisi quantitativa), quella ottenuta<br />
per aci<strong>di</strong>ficazione del campione a pH<br />
2 e quello libero; valutare se per una corretta<br />
misura dell’esposizione a benzene sia<br />
in<strong>di</strong>spensabile effettuare la determinazione<br />
dell’SPMA totale e quale sia il valore <strong>di</strong><br />
SPMA totale corrispondente al BEI <strong>di</strong> 25<br />
µg/g <strong>di</strong> creatinina stabilito dall’ACGIH.<br />
MATERIALI E METODI<br />
I campioni <strong>di</strong> urina vengono conservati<br />
congelati a -20°C fino al momento dell’analisi.<br />
Un’aliquota 3 ml viene ad<strong>di</strong>zio-<br />
nata con 1 ml <strong>di</strong> H 2 SO 4 9M e dopo 10 min con 1,6 ml <strong>di</strong><br />
NaOH 10 M per la determinazione dell’SPMA totale;<br />
una seconda aliquota viene portata a pH 2 con acido acetico<br />
glaciale; una terza utilizzata tal quale fornisce il valore<br />
<strong>di</strong> SPMA libero. Ai campioni così trattati vengono<br />
aggiunti 30 ng <strong>di</strong> S-Phenylmercapturic-3,3-d2 acid come<br />
standard interno per l’analisi quantitativa e sono poi<br />
sottoposti ad estrazione in fase solida su cartucce Seppack<br />
C18 da 500 mg precon<strong>di</strong>zionate con 3 ml <strong>di</strong> metanolo<br />
e poi 3 ml <strong>di</strong> acido acetico 0,1%. Si lava con 3 ml<br />
acido acetico 0,1% e si eluisce con 3 ml <strong>di</strong> metanolo. I<br />
campioni vengono analizzati in HPLC/MS/MS con sorgente<br />
Turbo Ion Spray su colonna SUPELCO Discovery<br />
RP C-18 150x4.6 mm, porosità 5 µm. La fase mobile è<br />
un gra<strong>di</strong>ente <strong>di</strong> metanolo e acido acetico 0,1% da 30:70<br />
v/v a 90:10 v/v, flusso 1 ml/min. La rivelazione è effettuata<br />
in modalità MRM in ioni negativi e le transizioni<br />
monitorate sono 238,1→109,1 per l’SPMA e<br />
240,1→109,1 per lo standard interno. Il tempo <strong>di</strong> ritenzione<br />
dell’SPMA e dello standard interno in queste con<strong>di</strong>zioni<br />
è <strong>di</strong> circa 7,7 min. L’analisi quantitativa viene effettuata<br />
me<strong>di</strong>ante una curva <strong>di</strong> calibrazione ottenuta con<br />
campioni <strong>di</strong> urina <strong>di</strong> un volontario non fumatore ad<strong>di</strong>zionati<br />
con concentrazioni <strong>di</strong> SPMA nel range 0,5 - 50<br />
ng/m e contenenti 10 ng/ml <strong>di</strong> standard interno. Il limite<br />
<strong>di</strong> rivelabilità e <strong>di</strong> quantificazione sono rispettivamente<br />
0,02 e 0,05 ng/ml. Con questo metodo sono stati<br />
analizzati 40 campioni <strong>di</strong> urina prelevati da soggetti sia<br />
fumatori che non, e per ciascuno viene calcolata la percentuale<br />
<strong>di</strong> SPMA misurata a pH 2 e senza correzione<br />
del pH (SPMA libero), rispetto alla quantità misurata<br />
per idrolisi quantitativa.<br />
RISULTATI<br />
I risultati ottenuti sono riportati nella tabella I.<br />
CONCLUSIONI<br />
La percentuale <strong>di</strong> SPMA determinata nei campioni<br />
analizzati a pH 2 è quasi il 45% del totale sui soggetti fumatori<br />
con una deviazione standard percentuale del 20%<br />
