of the left temporo- parietal lobe. The images also showed that the lesion caused a
moderate left to right mass affect with compression of the thalamus and left lateral
ventricle at the level of the hippocampus and rostral midbrain. There was also mild
compression of the left rostral colliculus of the midbrain consistent with herniation.
The high signal within the temporal lobe extended rostrally and caudally into the
white matter tracts into the frontal, parietal and occipital lobes consistent with
vasogenic edema. The T2W high signal lesion was hypointense to the brain
parenchyma on T1W imaging and had a faint intramedullary rim enhancement
pattern after gadolinium (0.1mmol/kg, IV) administration. There was also diffuse
enhancement of the deep planes of the temporal muscle. On T2* (GRE) imaging a
hypointense triangular lesion within the brain consistent with hemorrhage was
detected in the region of the skull fracture.
