needs <strong>to</strong> respond more proactively <strong>to</strong> disability. In a context of a health sec<strong>to</strong>r fac<strong>in</strong>g major<br />

challenges <strong>in</strong> achiev<strong>in</strong>g adequate coverage and quality of care for the entire population, it will be<br />

important <strong>to</strong> prioritize disability-related <strong>in</strong>terventions.<br />

17. Success <strong>in</strong> reduction of some communicable diseases has not been matched by<br />

improvements <strong>in</strong> the general public health system <strong>in</strong> areas which have significant impacts<br />

on disability. A large proportion of disabilities <strong>in</strong> <strong>India</strong> are preventable, <strong>in</strong>clud<strong>in</strong>g disabilities<br />

that arise from medical issues surround<strong>in</strong>g birth, from maternal conditions, from malnutrition,<br />

and from causes such as accidents and <strong>in</strong>juries. However, programs <strong>to</strong> address these are weak.<br />

One example is <strong>in</strong>effective programs for address<strong>in</strong>g micronutrient deficiencies, where <strong>India</strong><br />

ma<strong>in</strong>ta<strong>in</strong>s stubbornly poor nutritional outcomes (and where <strong>in</strong>terventions such as expand<strong>in</strong>g<br />

access <strong>to</strong> iodized salt or food fortification are highly cost effective). A second critical area of the<br />

general health system is reproductive, maternal and child health. Access <strong>to</strong> care dur<strong>in</strong>g<br />

pregnancy and delivery is poor <strong>in</strong> <strong>India</strong>. In the three years preced<strong>in</strong>g NFHS-2, 35 percent of<br />

pregnant women received no antenatal care: only a marg<strong>in</strong>al improvement on the early 1990s,<br />

and <strong>with</strong> high-risk groups still <strong>with</strong> less access <strong>to</strong> care. The hard-won battle aga<strong>in</strong>st some<br />

communicable diseases may also be <strong>in</strong> jeopardy. There appears <strong>to</strong> have been a worry<strong>in</strong>g decl<strong>in</strong>e<br />

<strong>in</strong> immunization rates at the national level. On a national basis, full immunization coverage has<br />

decl<strong>in</strong>ed from 54.2 <strong>to</strong> 48.5 percent <strong>in</strong> only 5 years.<br />

18. Policy and practice <strong>in</strong>dicate that the bulk of both prevention and treatment of<br />

disability will cont<strong>in</strong>ue <strong>to</strong> happen as part of the broader public and curative health delivery<br />

systems. The challenges of <strong>India</strong>n health systems have been discussed elsewhere and are outside<br />

the scope of this report, but are critical <strong>to</strong> prevention and <strong>to</strong> the lives of disabled people, who have<br />

generally greater needs for health system support. However, disability-specific <strong>in</strong>terventions are<br />

important also, and have received less attention. Generally, the focus of PWD-specific public<br />

<strong>in</strong>terventions has been rehabilitation. Technological support on rehabilitation is provided by the<br />

National Institutes on disability, though <strong>in</strong> recent years, <strong>India</strong> is <strong>in</strong>creas<strong>in</strong>g emphasis on<br />

community based approaches even <strong>in</strong> public <strong>in</strong>terventions. Perhaps the most <strong>in</strong>terest<strong>in</strong>g services<br />

be<strong>in</strong>g offered <strong>to</strong> people <strong>with</strong> disabilities are Community Based Rehabilitation (CBR), which has<br />

been effective <strong>in</strong> rural areas <strong>in</strong> address<strong>in</strong>g the primary care and therapeutic needs of PWD. The<br />

concept is <strong>in</strong>stitutionally flexible and can be operationalized by communities, NGOs and<br />

government, separately or <strong>in</strong> partnerships.<br />

19. The likelihood of seek<strong>in</strong>g health care differs across disabled groups. 5 Firstly,<br />

evidence from NSS shows that those disabled from birth are much less likely <strong>to</strong> seek care.<br />

Secondly, women <strong>with</strong> disabilities were somewhat less likely <strong>to</strong> seek care, and even less likely <strong>to</strong><br />

have assistive appliances. Regionally, access <strong>to</strong> care appears <strong>to</strong> be lowest <strong>in</strong> the North-East and<br />

eastern regions, while those <strong>in</strong> urban areas throughout <strong>India</strong> are much more likely <strong>to</strong> have sought<br />

care. As <strong>with</strong> the general population, higher levels of education substantially <strong>in</strong>crease the access<br />

<strong>to</strong> health care, as does co-residence of the person <strong>with</strong> disabilities <strong>with</strong> their parents.<br />

20. Many fac<strong>to</strong>rs drive low access <strong>to</strong> care among people <strong>with</strong> disabilities: (a) current<br />

disability identification and certification system functions poorly, <strong>with</strong> poor skills among<br />

providers, awareness among PWD low, and rural outreach poor; (b) the available evidence<br />

<strong>in</strong>dicates that provider attitudes rema<strong>in</strong> a constra<strong>in</strong>t on PWD access <strong>to</strong> health services; (c)<br />

community attitudes also cont<strong>in</strong>ue <strong>to</strong> be a constra<strong>in</strong>t on rais<strong>in</strong>g the profile of health services for<br />

PWD, particularly <strong>with</strong> mental illness; (d) physical accessibility of health facilities is a significant<br />

issue, and more acute at higher levels of the system; (e) as PWD households are poorer than<br />
