almost three times more likely <strong>to</strong> have had difficulties dur<strong>in</strong>g pregnancy, <strong>in</strong>dicat<strong>in</strong>g a much<br />
higher than average need for ante-natal care. They were also significantly less likely <strong>to</strong> have<br />
accessed government allopathic ante-natal care when they did (and more likely <strong>to</strong> have used<br />
public traditional or private providers).<br />
3.28. Disability aris<strong>in</strong>g from maternal causes is difficult <strong>to</strong> assess and estimate, but it is wellknown<br />
that low-birth weight, prematurity, maternal anemia, and malnutrition <strong>in</strong>crease the risk of<br />
disability among babies. For example, neural tube defects (NTD) a condition that leads <strong>to</strong><br />
considerable and irreversible disabilities <strong>in</strong> newborns can be prevented by <strong>in</strong>clud<strong>in</strong>g folic acid<br />
supplements <strong>in</strong> pregnant women’s diet. 66 The prevalence of NTD is reported <strong>to</strong> be 3.63 per 1000<br />
live births and the highest be<strong>in</strong>g <strong>in</strong> the northern states of Punjab, Haryana, Rajasthan and Bihar. 67<br />
3.29. Apart from impacts on the child, poor access <strong>to</strong> quality services can result <strong>in</strong> a range of<br />
disabl<strong>in</strong>g conditions for mothers themselves, some of which are easily treatable conditions such<br />
as fistula which can become disabl<strong>in</strong>g if not attended <strong>to</strong>. 68 Information on women <strong>with</strong><br />
disabilities (WWD) and their reproductive health needs <strong>in</strong> <strong>India</strong> is limited. 69 Among the most<br />
severe of these disabilities are the conditions result<strong>in</strong>g from obstructed labour, conditions<br />
virtually unknown <strong>in</strong> countries where Caesarean sections are easily available. Other l<strong>in</strong>ger<strong>in</strong>g<br />
problems may <strong>in</strong>clude anaemia, <strong>in</strong>cont<strong>in</strong>ence and sterility. Obstetric fistulae disproportionately<br />
affect very young and very poor women. Internationally, girls under 15 are 25 times more likely<br />
<strong>to</strong> have critical complications related <strong>to</strong> pregnancy than women <strong>in</strong> their 20s. 70<br />
3.30. (f) Prevention outside the health care system: road traffic <strong>in</strong>itiatives: As<br />
noted, accidents are a major cause of disability, and traffic accidents account for a significant<br />
share of all accidents. While this is <strong>in</strong>creas<strong>in</strong>gly acknowledged <strong>in</strong> <strong>India</strong>, efforts <strong>to</strong> address the<br />
epidemic of traffic accidents have been more limited. Recently, GoI has produced a draft<br />
National Road Safety Policy which sets a number of positive directions for action. However,<br />
execution of policy is largely a matter for the states, and concrete action has been lack<strong>in</strong>g <strong>in</strong> many<br />
cases. Kerala provides a positive example of state-level action which addresses a range of fac<strong>to</strong>rs<br />
<strong>in</strong> road safety, <strong>with</strong> a state-level Road Safety Action Plan developed, and <strong>in</strong>itial implementation<br />
begun through <strong>in</strong>itiatives such as a Good Practice Manual of Public Education <strong>in</strong> Road Safety.<br />
Pilots are under discussion <strong>in</strong> the Coch<strong>in</strong>/Ernakulam area for model road safety programs<br />
<strong>in</strong>clud<strong>in</strong>g promotion of seat belt and helmet use. There has also been focus on police accident<br />
report<strong>in</strong>g practices, vehicle fitness <strong>in</strong>itiatives, and emergency medical response <strong>to</strong> accidents.<br />
Tamil Nadu is also tak<strong>in</strong>g action such as mapp<strong>in</strong>g of “accident black spots”, <strong>with</strong> planned<br />
<strong>in</strong>vestments <strong>in</strong> both improv<strong>in</strong>g road safety and <strong>in</strong> medical response capacity <strong>in</strong> high-risk areas.<br />
C. Curative and rehabilitative health <strong>in</strong>terventions and PWD<br />
3.31. As noted <strong>in</strong> Chapter 7, the health sec<strong>to</strong>r is the one where the PWD Act makes the<br />
weakest <strong>in</strong>cremental commitments <strong>in</strong> public policy. This is for two ma<strong>in</strong> reasons. Firstly, it<br />
focuses ma<strong>in</strong>ly on prevention and early detection of disabilities, and rais<strong>in</strong>g of public awareness<br />
