Seit Juni 2006 ersetzt eine mittels Melt Extrusion Technik hergestellte LPV/r-Tablette (a 200/50mg) die ursprüngliche Weich-Gelatine-Kapsel (SGC) (a 133/33mg). Dieses ermöglichte eine Senkung der Tablettenlast von 6 SGC/d auf 4 Tabl./d. Die Medikamenteneinnahme kann nun nahrungsunabhängig erfolgen und die Tabl. bei Raumtemperatur gelagert werden. Eine häufige NW der SGC sind gastrointestinale Beschwerden, v.a. Diarrhöen. Fragestellung: Mittels eines standardisierten Fragenkatalogs wurde untersucht, ob sich Unterschiede hinsichtlich der zwei Darreichungsformen in dem für die Adhärenz wichtigen Einnahmekomfort der Medikation, der Befindlichkeit des Patienten und den unerwünschten Wirkungen ergeben. Methodik: Bei 27 konsekutiven Patienten mit LPV/r-SGC-Therapie wurde eine schriftliche Befragung bestehend aus 10 Items (geschlossene Fragen) durchgeführt. Nach Therapieumstellung auf Meltrex-LPV/r erhielten die Pat. denselben Fragebogen. 18 Pat. hiervon erhielten einen erweiterten Fragebogenteil mit 5 Items. Ergebnisse: 100% (n=27) der Patienten waren mit der SGC- Therapie, 93% (25) mit der Tabl.-Therapie zufrieden. Keine NW hatten mit der SGC 70% (19), mit der Tabl. 78% (21). Unter SGC-Therapie hatten 67% (18) 1-3x/d Stuhlgang und 48% (13) litten nie unter Durchfall, während unter Tabl.-Therapie 78% (21) 1-3x/d Stuhlgang hatten und 44% (12) nie unter Durchfall litten. 56% (15) unter SGC-Einnahme gaben an, keine Dosis vergessen zu haben, während 63% (17) dies unter Tabl.-Einnahme angaben. 70% (19) der Patienten mit SGC „fühlten sich gut“, während 60% (16) mit Tabl. dies angaben. Die veränderte Pillenzahl fanden 84% (15) „sehr gut“; die einfachere Lagerung beurteilten alle Pat. als „sehr gut“. 88% (16) bevorzugten die neue Tabl. Als Grund wurde die Reduktion der Pillen sowie die Lagerung genannt. Diskussion: Bezüglich der NW oder der Befindlichkeit wurde die LPV/r-Meltrexformulierung als mindestens gleich gut im Vergleich zur LPV/r-SGC beurteilt. Von den meisten Patienten wurde die neue LPV/r-Meltrex-Tabl. bevorzugt, v. a. wegen der reduzierten Pillenzahl und der verbesserten Lagerung. D.37 (Vortrag) Detection <strong>of</strong> abacavir hypersensitivity by ELISpot method Esser S. 1 , Jablonka R. 1 , Heinemann F. 2 , Grabbe S. 1 , Grosse-Wilde H. 2 , Lindemann M. 2 1 Universitätsklinikum Essen, Klinik für Dermatologie und Venerologie, Essen, Germany, 2 Universitätsklinikum Essen, Institut für Immunologie, Essen, Germany Objective: Abacavir (ABC) causes a potentially fatal hypersensitivity reaction (HSR) in 5-8% <strong>of</strong> HIV-infected patients occurring usually within the first 6 weeks after starting treatment. HSR is diagnosed clinically and, actually, the testing for HLA-B*5701 becomes available as a further predictive test. The correlations between the occurrence <strong>of</strong> HSR, patch test reactions to ABC, and HLA-B*5701 are evaluated in the PREDICT Study. A safe diagnostic tool to confirm ABC-HSR is mandatory. Therefore, we here evaluated an ELISpot method to measure cellular in vitro responses to ABC. Methods: ABC-ELISpot was performed in HIV-infected patients with clinically determined HSR to ABC who had stopped the ABC treatment for more than 6 weeks (n=11) and HIV-patients taking ABC for more than 12 weeks without suspected HSR (control group, n=13). Twenty <strong>of</strong> the HIV-patients were male, 4 female, the average CD4 T cell count was 338/ml (range 26-954). Peripheral blood mononuclear cells from all patients were stimulated by 1, 4, and 10 mg/mL <strong>of</strong> abacavir sulfate in 200 mL <strong>of</strong> cell culture medium. Following preincubation for 2 days, the cells were incubated in ELISpot plates for 2 further days and the production <strong>of</strong> interferon-gamma producing cells was measured. Results: Patients with vs. without HSR displayed a significantly higher number <strong>of</strong> ABC-specific cells (7.6±4.7 vs. - 0.2±1.1 spots increment, P=0.03). Using a threshold <strong>of</strong> 2 spots increment - which was determined by discrimination and receiver operating curve (ROC) analyses - HSR could be predicted with a sensitivity <strong>of</strong> 64% and a specificity <strong>of</strong> 85%. Of note, 2 patients were HLA-B*5701 positive, both suffered from HSR, and both were positive to the ELISpot. [Abacavir ELISpot] Conclusions: We established the first ABC-specific ELISpot which appears as an additional tool to detect HSR retrospectively. Its application could avoid the risk <strong>of</strong> life-threatening reactions during rechallenge with ABC in HIV-patients with vague suspicion <strong>of</strong> HSR.
