Services of a Message Therapist will be covered up to an annual maximum of $300.00. Vision care maximum $300.00 every 24 months in addition to eye examinations every 24 months, and hearing aid acquisition every 36 months. Orthodontics will be provided as a 50/50 co-‐pay to a maximum of $1,500.00 per insured member. Coverage as per standard insurer policy. DATED this day of 200 .
