CONTENTS OF HEADQUARTERS FILES<br />

1. Every Marine has an official military personnel file (OMPF) at<br />

HQMC which provides a history of the Marine's career from entrance<br />

into the Marine Corps or the Marine Corps Reserve, until severance<br />

from the Marine Corps or the Marine Corps Reserve by discharge,<br />

retirement, resignation, or death. Administrative materials or<br />

documents to support the chronology of events for the Marine's<br />

history is incorporated in the OMPF at HQMC, as well as field<br />

service records, as required. The authority for insertion of these<br />

materials into the respective records is provided by current Marine<br />

Corps directives. As needs and records management techniques<br />

change, certain forms may become obsolete and be replaced by new<br />

forms or automated recording procedures. When this occurs,<br />

documents filed as a result of the old requirement will not be<br />

removed from either the OMPF or the field service record.<br />

2. All material forwarded to the CMC (MMSB-20) for inclusion in<br />

the OMPF must contain the grade, full name, and SSN. The SSN<br />

format prescribed for official correspondence is appropriate;<br />

however, recruit computer-generated SRB pages are produced in a<br />

different format, 123 45 67 89 vice 123 45 6789. Either format is<br />

acceptable for use in the field service records and for<br />

identification of material for inclusion in the OMPF.<br />

3. The OMPF is structured into the following areas:<br />

a. Service Folder (TABLE 1-1). Consists of contract<br />

information, discharge documents, reserve documents, orders to<br />

active duty, and general administrative and service documents<br />

used to compute service time for benefits, programs, or<br />

retirement.<br />

b. Commendatory/Derogatory Folder (TABLE 1-2). This folder<br />

contains documents on civilian and military education, personal<br />

awards information, courts-martial/nonjudicial punishments, and<br />

other material reflecting significant personal achievement or<br />

adversity that is pertinent to making decisions for purposes of<br />

selection, assignment, and retention.<br />
