(3) Block 2. Place an "x" in the block reflecting the<br />

type of court-martial by which the accused was tried. For date<br />

of trial, reflect the date the sentence was adjudged.<br />

(4) Block 3. State each charge and describe each<br />

specification in sufficient detail to show date and place of<br />

commission, and nature and seriousness of offense alleged. The<br />

mere statement, "Viol of Art. 86" is not adequate. For example,<br />

in case of UA offenses, show the inclusive dates of absence; in<br />

the case of disobedience of orders, include the gist of the order<br />

violated so that the seriousness of the offense is indicated.<br />

Example: "Chg I-Viol Art 134 UCMJ. SPEC 1. Drunk and<br />

disorderly on 960605 in Bremerton, WA. SPEC 2. Dis M16 rifle in<br />

bks; MB, NSB, Bangor, Bremerton, WA, on 960605. Chg II-Viol Art<br />

85 UCMJ. SPEC 1., etc." Number charges and specifications as<br />

they were numbered on the charge sheet.<br />

(5) Block 4. State the findings of each charge and each<br />

specification under each charge. For example, when there is only<br />

one charge and specification, the findings would be set out as<br />

follows:<br />

"Chg: G; Spec, Chg: G"<br />

When there is more than one charge and specification, the<br />

findings would be set out as follows:<br />

"Chg I: G; Spec 1, Chg I: G; Spec 2, Chg I: G;<br />

Chg II: NG; Spec, Chg II: NG;<br />

Chg III: G; Spec 1, Chg III: NG; Spec 2, Chg<br />

III: G; Spec 3, Chg III: NG"<br />

When the accused is found guilty of an offense that is different,<br />

or in the case of unauthorized absence, shorter than was alleged<br />

in the specification, use this example:<br />

"Chg I: G; Spec 1, Chg I: G-UA (AWOL) fr<br />

neck and" substituting "in the back and"<br />
