administrative remarks page entry. (See paragraph

special courts-martial promulgating orders per JAGINST 5800.7,
JAGMAN.

considered necessary by the commander.
ee. Certified copy of separation physical (medical and
dental).
3002. COVER -- OFFICER’S QUALIFICATION RECORD (NAVMC 123A).
Prepare an OQR cover for every officer record per paragraph
2004.2b(1). Reporting units may affix a white gummed-label
produced through the Commander's Unit Diary Data Base retrieval
system (Marine Integrated Personnel System (MIPS)) or word
processing system with label producing capabilities. Gummedlabels
will contain only the identifying information indicated
above. Any additional information is prohibited. When the label
is used, affix it in the space provided for the last name.
