(e) Record of Emergency Data - file on the right side of the<br />
SRB. If information on the form is changed, report corrections per<br />
<strong>MCO</strong> P1080.40, MCTFSPRIM, as appropriate.<br />

Election and Certificate (2d page) - transfer to the new SRB,<br />
unless Marine opts to change beneficiaries. If a new form is<br />
created, distribute copies per <strong>MCO</strong> P1741.8.<br />
(3) When a permanent Warrant Officer accepts a temporary<br />
commission or a Reserve officer accepts a permanent appointment in<br />
the Regular Marine Corps, retain the OQR. Move the old NAVMC 763<br />
to the document side of the record. Place the new NAVMC 763 as the<br />
top document on the standard side.<br />
b. Upon change from enlisted to officer status, dismantle the<br />
SRB. Extract all documents pertinent to the OQR (see paragraph<br />
3001.3) and file them on the left side of the newly created OQR.<br />
Deliver those documents, including the ROS, which need not be<br />
transferred to the new OQR to the Marine. Dispose of standard<br />
pages as follows: Insert the Enlistment/Reenlistment Document and<br />
any extensions thereto, pages 12, 13, 13A, and OPNAV Form 1326/2 on<br />
the document side of the OQR; transfer pages 3, 11, the final<br />
computer-generated RED (Sergeant and above), and SGLI Election Form<br />
to the standard side of the OQR. Destroy the SRB cover.<br />
4. Making Entries (General). Make all entries with a typewriter<br />
or word processor, if possible. Use of the self-correcting<br />
typewriter is permitted in making OQR and SRB entries provided the<br />
finished entry is clear, neat, and does not appear as an<br />
unauthorized/indistinct entry alteration. If not possible to use a<br />
typewriter, print entries neatly in black ink.<br />
a. Use black ribbon for all typewritten entries and black ink<br />
for all rubber stamp and handwritten entries. Signatures must be<br />
made in the Marine's handwriting and not printed.<br />
b. Rubber stamps may be used.<br />
c. Erasures, strike overs, and/or any type of correction<br />
fluids or ink eradicators are prohibited.<br />
