k. Waiver of Designation as Sole Surviving Son/Daughter and<br />

Revocation of Waiver. Marines designated sole surviving<br />

son/daughter as defined by <strong>MCO</strong> P1300.8, may waive or revoke<br />

privileges as appropriate by signing one of the following sample<br />

entries and submitting same signed statement to CMC (MMOA).<br />

(1) Waiver of privileges:<br />

“(Date). I hereby waive my rights to restrictive<br />

assignment as a sole surviving son/daughter per <strong>MCO</strong> P1300.8."<br />

(2) Revocation of waiver:<br />

“(Date). I hereby revoke the waiver of my sole<br />

surviving son/daughter status and request reinstatement of that<br />

status per <strong>MCO</strong> P1300.8."<br />

l. Confirmed Incidents of Illegal Drug Abuse or Possession.<br />

(Refer to <strong>MCO</strong> P5300.12) Record in the OQR all confirmed incidents<br />

(civil or military) of illegal drug involvement. Consult <strong>MCO</strong><br />

P1610.7 concerning requirements for submission of a "DC" fitness<br />

report. Whenever the counseling entry is made, afford the Marine<br />

the opportunity to make a written rebuttal, if any, for inclusion<br />

on the document side of the Marine's OQR. The entry must include<br />

the language contained in paragraph 3005.2d concerning the<br />

officer's right to submit a rebuttal. The Marine will acknowledge<br />

(sign) the counseling entry. Sample entry:<br />

“(Date). Counseled this date concerning my illegal drug<br />

involvement; specifically, indicate involvement; i.e., trafficking,<br />

possession, usage and, (if identified through urinalysis<br />

testing, give DoD Drug Testing Laboratory message number and type<br />

of drug identified). Include specific recommendations for<br />

corrective action, to include date placed on/removed from the<br />

Urinalysis Surveillance Program (USP) and date Marine was seen by<br />

a substance abuse counselor. I am advised that processing for<br />

administrative separation for cause is mandatory per <strong>MCO</strong><br />

P1900.16. I was advised that within 5 working days after<br />

acknowledgment of this entry, a written rebuttal can be submitted<br />

and this rebuttal will be filed on the document side of the OQR.<br />

I choose (to) (not to) make a statement."<br />

m. Alcohol Abuse. (Refer to <strong>MCO</strong> P5300.12) Commanders will<br />

have an entry made in the OQR after counseling a Marine for a<br />

first or subsequent alcohol-related incident that resulted in<br />
