NOTES:<br />
1. Actions involving immediate reenlistments into the Marine<br />
Corps or Marine Corps Reserve are accomplished by the unit<br />
holding the Marine's SRB.<br />
2. There is no requirement to prepare a DD Form 1966,<br />
Application for Enlistment--Armed Forces of the United States, on<br />
immediate reenlistments in the Marine Corps or Marine Corps<br />
Reserve.<br />
3. DD Form 1966, Application for Enlistment--Armed Forces of the<br />
United States, must be prepared for all original enlistments and<br />
reenlistments in the Marine Corps Reserve as a result of broken<br />
service (refer to <strong>MCO</strong> P1040.31 and <strong>MCO</strong> P1040R.35 for definition<br />
of continuous/broken reenlistment).<br />
4. Forward a reproduced copy of any rebuttal statement(s) to a<br />
derogatory administrative remark page 11 entry(ies) along with<br />
the page 11 which contains the entry.<br />
4004. <strong>CH</strong>RONOLOGICAL RECORD (NAVMC 118(3))<br />
1. This is a standard page designed for recording in<br />
chronological sequence, the events or duties performed by a<br />
Marine from time of commissioning or enlistment, through and<br />
including, complete severance from the Marine Corps or Marine<br />
Corps Reserve. The chronological record of a Marine is important<br />
because it is a historical recording of assignments documented in<br />
the sequence of occurrence. Beginning with recruit training/OCS,<br />
maintain the chronological record following the Marine's<br />
assignment sequence using the prescribed NAVMC form. Whenever<br />
page 3 is full, or a new version is prescribed, prepare a<br />
supplemental page per the instructions contained in paragraph<br />
2004.2a. Line out the unused portion of the page to preclude<br />
further entries after the supplemental page has been added.<br />
Place the supplemental page behind the full or closed-out pages<br />
in the appropriate labeled sequence, to include any NAVMC 118(3)<br />
(Rev. 6-62) or earlier versions of this form which have a<br />
combination of chronological information, general military<br />
subjects (essential subjects), and conduct and duty proficiency<br />
markings that are now automated and reported by unit diary entry.<br />
