"(Date). Preliminary PRP screening of personnel<br />

records, medical evaluation, and personnel interview IAW<br />

SECNAVINST 5510.35. Appropriate investigation initiated. You<br />

are suitable for transfer for training. Capt. F. H. CLAYTON,<br />

USMC, SOI, 5Jun94." (Signature of CO or official designated by<br />

CO.)<br />

(2) Formal PRP screening is required for personnel engaged in<br />

training leading to a PRP assignment. The training command will<br />

review the individual's personnel records to ensure that the<br />

preliminary screening was done, and if the individual is not<br />

qualified based on this record review alone, the training command<br />

will briefly state that the member was found unsuitable for PRP<br />

training assignment. Sample entry:<br />

"(Date). Formal<br />

PRP screening has been conducted and based a upon review of<br />

records, you were found unsuitable for PRP training and<br />

assignment IAW SECNAVINST 5510.35. Maj J. M. CARRIA, USMC."<br />

(Signature of CO or official designated by CO.)<br />

(3) Permanent decertification is a formal determination<br />

that the Certifying Official can no longer assert that the<br />

individual continues to meet the reliability standards provided<br />

in enclosure (3) to SECNAVINST 5510.35. The following signed and<br />

dated entry will be made:<br />

"(Date). I, (name/grade/MOS), have been advised that<br />

I have been permanently decertified for assignment to a<br />

reliability billet as defined in SECNAVINST 5510.35. I<br />

understand this decertification will be made a part of my<br />

permanent record. I did/did not submit a statement on my behalf<br />

with the report of decertification submitted to CMC.<br />

c. Record of Issue of Wet-Type Diver's Dress, Campaign<br />

Service Hat, Suitcase and Trunks. An entry is required when a<br />

Marine is issued any of the above items. The entry is considered<br />

permanent. Do not change or delete the entry unless the Marine<br />

is separated under other than honorable conditions, and the<br />

equipment is returned.<br />
