SERVICE RECORD BOOK (SRB)<br />
4000. INTRODUCTION. A SRB consists of a cover, standard pages,<br />
and documents. Assemble the SRB as indicated in the following<br />
subparagraphs.<br />
4001. CONTENTS OF THE SERVICE RECORD BOOK (SRB)<br />
1. Standard Pages. Prepare and insert the following standard<br />
pages on the right side of the record: DD Form 4, Enlistment/<br />
Reenlistment Document and signed statements of understanding<br />
which are stapled to the service record copy of the form,<br />
followed by pages 3, 11, RED following and Forms SGLV-8285 and/or<br />
SGLV-8286. NAVMC 10132 pages 13 and 13A are prepared and<br />
inserted when required. File any NAVMC 321a, Agreement to Extend<br />
Enlistment, when executed, on top of the enlistment/ reenlistment<br />
document which it extends. Maintain documents prescribed for<br />
retention in the SRB on the document side. Recruit depots will<br />
ensure that before transfer of recruits, standard pages 3, and<br />
11, plus other required pages are part of the SRB. Enter last<br />
name, first name, middle initial and SSN at the bottom of each<br />
page per paragraph 2004.2a. (See paragraphs 4006 through 4012<br />
regarding entries to be made on individual pages.) When any page<br />
is filled or there is not enough room for recording further<br />
entries in one or more sections, note at the bottom of the page<br />
"Continued on supplemental page" per paragraph 2004.2a. Place<br />
the supplemental page behind the closed-out pages in the<br />
appropriately labeled sequence.<br />
2. Documents. The document side of the SRB is provided for<br />
inserting superseded standard pages from the service record,<br />
official letters, certificates, and various other documents<br />
regarding the Marine that should be made a permanent part of the<br />
official record. Only file information that is relevant and<br />
necessary to accomplish a purpose required by statute, Executive<br />
Order, Marine Corps Order, or other directive from higher<br />
headquarters. Do not use the SRB as a catchall. Documents of<br />
temporary value, which are not specifically required to be filed<br />
in the SRB, are filed in the unit's correspondence files; do not<br />
file them in the SRB. Extra copies of transfer orders, local<br />
command memoranda, endorsement correspondence, or other<br />
extraneous papers are not authorized for retention. Insert the<br />
below listed documents, if applicable.<br />
