j. Early Separation Upon Return to the CONUS. The following<br />
entry is required when a Marine requests early separation upon<br />
return to CONUS per <strong>MCO</strong> P1900.16, par 1006. Sample entry:<br />
"(Date). I hereby consent to be (discharged) (released)<br />
on (Date) in lieu of my normal or established date of discharge<br />
or release on (Date). My enlistment (including any extension<br />
thereof) or period of extended active duty will expire 90 days or<br />
less after the date of my arrival in the CONUS. I am not<br />
indebted to the U.S. Government. I do not intend to reenlist. I<br />
am not transferring to the Fleet Marine Corps Reserve. I<br />
understand that entitlement to pay and allowances and credit for<br />
active Federal Service ceases on the actual date of my separation<br />
from active duty. (Signature of Marine)."<br />
k. Voluntary withdrawal from Enlistment Incentives Program.<br />
An entry is required when a Marine, prior to completion of Marine<br />
Combat Training, voluntarily withdraws from an enlistment<br />
incentive program. Sample entry:<br />
"(Date). I have voluntarily withdrawn as an enlistee<br />
under my enlistment incentives program (Program Enlisted For<br />
(PEF) code). I understand that I will now be assigned to any<br />
MOS, per the needs of the Marine Corps, and that the MOS I am<br />
assigned may be a MOS within my original enlistment program.<br />

l. Certification and Suspension or Revocation of Air Traffic<br />
Control (ATC) Certification. NAVAIR 00-80T-114, paragraph<br />
3.3.1.2, requires a permanent record of ATC ratings. Accordingly,<br />
make an entry upon initial certification of ATC ratings. Local<br />
ATC recertifications do not require additional entries in the<br />
service record. The initial certification entry is in addition<br />
to the entries required in the Marine's ATC training and<br />
qualification record (NAVMC 11004). NAVAIR 00-80T-114, paragraph<br />
4.4, requires an entry in the service records of Marines whose<br />
certification as an ATC is suspended or revoked. NAVAIR<br />
00-80T-114 further stipulates that a suspension or revocation is<br />
an administrative action and is not to be construed as<br />
disciplinary action. The entry must contain that information as<br />
part of the entry. The Marine will sign the<br />
suspension/revocation entry. Sample entry:<br />
