will initial any erasures or corrections on all copies of the<br />

document. Erasures or corrections to item 5-"Date of<br />

Enlistment/Reenlistment," item 8-(service period of enlistment,<br />

and pay grade), or any item(s) in Section D-"Certification and<br />

Acceptance" and Section E-"Confirmation of Enlistment or<br />

Reenlistment," are prohibited. Errors in these items will<br />

necessitate a complete retype of the document.<br />

b. Correct errors discovered after the immediate<br />

reenlistment is effected per the instructions contained in<br />

chapter 5 of this Manual.<br />

c. The service representative who accepts the Marine for<br />

immediate reenlistment will, prior to signing the completed form,<br />

verify typed entries on all copies and explain all applicable<br />

parts of the document to the Marine.<br />

d. Enter all dates on the DD Form 4 in year, month, day<br />

format. Dates will show the last two digits of the year, the two<br />

digit month, and the day shown in two digits. For example:<br />

960704.<br />

3. Detailed Instructions. The following preparation<br />

instructions apply to immediate reenlistments in the Marine Corps<br />

and Marine Corps Reserve only. Contracts prepared on initial<br />

enlistees at the Military Entrance Processing Stations have<br />

separate completion instructions.<br />

a. SECTION A - ENLISTEE/REENLISTEE IDENTIFICATION DATA<br />

(1) Item 1 -- Name. Enter full last name (including<br />

compound name, if applicable; i.e., mother's surname for those<br />

Marines who use it in combination with father's family name),<br />

first name, middle name(s), and any suffixes such as Jr., Sr.,<br />

III, etc. If the Marine was given an initial or initials rather<br />

than first and/or middle name, enter such initial or initials.<br />

If there is no middle name or initial, make no entry for the<br />

middle name. Do not use punctuation of any sort including<br />

apostrophes, periods, or hyphens. Spaces will not be inserted<br />

between sections of compound names nor used as a substitute for<br />

apostrophes or hyphens. For example:<br />
