OFFICER'S QUALIFICATION RECORD (OQR)<br />

3000. INTRODUCTION. An OQR consists of a cover, standard pages,<br />

and documents. Assemble the OQR as indicated in the following<br />

subparagraphs.<br />

3001. CONTENTS OF THE OFFICER’S QUALIFICATION RECORD (OQR).<br />

1. Prepare an OQR cover for every officer’s record per paragraph<br />

2004.2b(1). Insert authorized standard pages (see paragraph<br />

3001.2 below) on the right side of the cover. File any other<br />

matter authorized for filing in the OQR as a document on the left<br />

side.<br />

2. Standard Pages. Insert standard pages in the OQR Cover<br />

(NAVMC 123a), beginning with NAVMC 763, Appointment Acceptance<br />

and Record, as the top document on the right side and followed in<br />

order by the standard pages listed below. It should be noted<br />

that some of the standard pages may have originated from the<br />

officer's prior enlisted record and may be brought forward as<br />

part of a continuing record upon acceptance of commission (see<br />

paragraph 4001.1).<br />

a. NAVMC 118(3) (Rev. 5-74), Chronological Record<br />

b. NAVMC 118(11) (Rev. 3-82), Administrative Remarks.<br />

c. Computer-generated Record of Emergency Data (RED) (NAVMC<br />

10526/DD Form 93 or locally produced form (Reserve or officer<br />

candidate) are acceptable until accession into MCTFS.)<br />

d. SGLV - 8285, Request For Insurance (SGLI) (see <strong>MCO</strong><br />

P1741.8).<br />

e. SGLV - 8286, Servicemember's Group Life Insurance<br />

Election and Certificate (see <strong>MCO</strong> P1741.8).<br />

3. Documents. The document side of the OQR is provided for<br />

inserting some standard pages from the enlisted service record<br />

and various documents regarding the Marine that should be made a<br />

permanent part of the official record. Only file information<br />

that is relevant and necessary to accomplish a purpose required<br />

by statute, Executive order, Marine Corps order, or other directive<br />

of higher headquarters. Do not use the OQR as a catchall.<br />

Documents of temporary value, which are not specifically required<br />
