is not creditable for pay purposes. Local command changes,<br />
additions, or deletions in this section are prohibited. Any<br />
information listed under "Prior Service" which is thought to be<br />
in error will be corrected by the CMC (MMSB-10) and will require<br />
specific documentation by the local command to justify any<br />
change. The only exception to the above is if an item is left<br />
blank and separate instructions are given for completion (i.e.,<br />
Item 21D left blank for date discharged from last period of<br />
service). In most cases, the date of discharge for the last<br />
period of service will be the date prior to the date of acceptance<br />
in item 22.<br />
d. Item 22 - Administration of Oath. After the oath of<br />
office has been given, the appointee will sign their full name on<br />
the signature line of the original. The NAVMC 763 is printed on<br />
self-carbonizing paper that will enable the signature from the<br />
original to be transferred to all attached copies. Each copy of<br />
the NAVMC 763 will be certified as a true copy by the appointee's<br />
original signature (not the officer administering the oath) in<br />
the certification area provided on the form. Special care should<br />
be taken to ensure that a carbonized signature does not appear in<br />
the certification portion of any copy. Type the organization/<br />
location at which the appointment is taking place on the line<br />
following: "Subscribed and duly sworn to before me at unit." For<br />
example: MCCDC, QUANTICO, VA. Enter the date as 1ST, 2D, 3D,<br />
etc., JAN, FEB, MAR, ETC., and 98, 99, etc. The officer administering<br />
the oath will sign the original copy on the line provided.<br />
All remaining copies must bear the carbonized signature of the<br />
appointing officer. The name, grade, and component of the<br />
officer administering the oath will be typed below the signature<br />
line.<br />
e. EFF DATE - Effective Date. The effective date is used<br />
primarily for reversions and resignations when the officer<br />
departs the area and must sign item 22 prior to the effective<br />
date. In every instance in which "effective date" is used, the<br />
CMC (MCRC) will provide specific authority. In all other cases,<br />
the effective date is the date the NAVMC 763 is signed as entered<br />
in item 22, regardless of any date entered in the "effective<br />
date" that was entered without proper authority.<br />
4. The commander of the organization designated to administer<br />
the oath of office will immediately make distribution of the<br />
completed NAVMC 763, as follows:<br />
