(4) "(Date). Mr/Mrs (full name) personally counseled<br />
this date on provisions of SBP and of the effect of her husband/<br />
his wife electing (no coverage, child coverage, or reduced<br />
coverage) and DD Form 2656 certification completed.” If personal<br />
counseling is not feasible, indicate that it was accomplished by<br />
mail and that the DD Form 2656 was forwarded for certification<br />
and return. (Attach the certified mail return receipt to the<br />
page 11 entry.)<br />
(5) "(Date). Marine, single with no dependent children,<br />
elected insurable interest coverage on behalf of (full name), DD<br />
Form 2656 forwarded to DFAS, Cleveland this date. Signature of<br />
Marine."<br />
w. Withholding Authority for the Marine Corps Good Conduct<br />
Medal (GCM). Make the following entry when a Marine is denied<br />
awarding of the GCM per the letter from the officer exercising<br />
general courts-martial authority. Sample entry:<br />
“(Date). I have been informed by my commanding officer<br />
that I am not eligible for the awarding of the Good Conduct<br />
Medal, award period of (enter appropriate award period) per the<br />
authority contained in the CG,(Appropriate MSC) letter dated<br />
_______________.”<br />
x. Additional Guidance Concerning Page 11 Entries:<br />
(1) Commanders are authorized to make other entries on<br />
this page that are essential to document an event in a Marine's<br />
career for which no other means or method of recording exists.<br />
Authorized entries under this rule must meet two tests: (1) the<br />
information contained in the entry is of permanent value to the<br />
Marine's career; and, (2) the information is not, or cannot be,<br />
documented anywhere else in the SRB, medical records, or the<br />
Marine's MCTFS record. Participation in certain operations<br />
(excluding routine training exercises/operations) not<br />
specifically identified as combat operations or as having a<br />
potential for escalating into combat operations and humanitarian<br />
4-57
