3005 IRAM<br />
(3) Permanent decertification is a formal determination<br />
that the Certifying Official can no longer assert that the<br />
individual continues to meet the reliability standards provided<br />
in enclosure (3) to SECNAVINST 5510.35. The following signed and<br />
dated entry will be made:<br />
“(Date). I, (name/grade/MOS), have been advised that<br />
I have been permanently decertified for assignment to a reliability<br />
billet as defined in SECNAVINST 5510.35. I understand this<br />
decertification will be made a part of my permanent record. I<br />
(did) (did not) submit a statement on my behalf with the report<br />
of decertification submitted to CMC."<br />

e. Voluntary Retirement of Officers on Active Duty. An<br />
entry is required when an officer on Active Duty requests a<br />
transfer to the retired list. The Marine will sign the entry.<br />
Sample entry:<br />
“(Date). I request transfer to the retired list effective<br />
(Date). I have signed the Commanding Officers<br />
pre-application checklist and fully understand the ramifications<br />
of this request."<br />
f. Request for Resignation. An entry is required when an<br />
officer requests resignation. The Marine will sign the entry.<br />
Sample entry:<br />
"(Date). I request to resign my commission in the Marine<br />
Corps effective (Date) and (do) (do not) desire a Reserve commission.<br />
I have been counseled per paragraph 5003 of <strong>MCO</strong> P1900.16.”<br />
g. Record of Issue of Wet-Type Divers' Dress, Campaign<br />
Service Hat, Suitcase, Trunks, and General Officer Personal<br />
Distinguishing Flap. Make an entry when a Marine is issued any<br />
of the above items. The entry is permanent. Do not change or<br />
delete the entry unless the Marine is separated under other than<br />
honorable conditions, and the equipment is returned.<br />
h. Certification and Suspension or Revocation of Air Traffic<br />
Control (ATC) Certification. NAVAIR 00-80T-114, paragraph<br />
