c. Performance Folder (TABLE 1-3). Contains fitness reports<br />

and standard addendum pages for all sergeants and above. The<br />

Performance Folder may contain memoranda originated by the CMC<br />

(MMSB) as necessary to record historical or administrative<br />

information.<br />

d. Field Folder OQR/SRB (TABLE 1-4). Upon separation from the<br />

Marine Corps, documents contained in the Service Record<br />

Book/Officer Qualification Record (OQR/SRB) are filed in the<br />

Service Folder. If a Marine has periods of broken service,<br />

documents from previous OQR/SRBs will be contained in the Field<br />

Folder. This Table contains a partial listing of documents and<br />

standard pages suitable for inclusion in the OMPF.<br />

e. Health Folder (TABLE 1-5). Unless a Marine had broken<br />

service which ended prior to 1994, or is presently on the Temporary<br />

Disability Retired List (TDRL), no Health Folder is maintained.<br />

Since 1994, health and dental records are sent to the Department of<br />

Veterans Affairs, Service Medical Records Center (SMRC), P.O. Box<br />

150950, St. Louis, MO 63115 upon discharge.<br />

4. Certain limitations exist regarding the kinds of material<br />

authorized for inclusion in the OMPF and the specific locations<br />

(data areas) within the record. The following guidelines apply in<br />

determining whether or not certain documents are placed in the<br />

OMPF.<br />

a. Extraneous Material. The CMC (MMSB) will not file material<br />

which is not relevant to the documentation of the Marine's history<br />

of service.<br />

b. Duplicate Material. The CMC (MMSB) will destroy material<br />

identified as duplicate of documents already in the OMPF without<br />

notifying the forwarding command or individual.<br />

c. Adverse Matter. Any document that reflects unfavorably<br />

upon a Marine's mental, moral, or professional qualifications.<br />

Subject to the following rules, all relevant adverse material is<br />

filed in the OMPF.<br />

(1) Fitness Reports. Article 1122, U.S. Navy Regulations,<br />

1990, provides that adverse material shall not be placed in officer<br />

or enlisted fitness reports, or appended as an addendum, unless the<br />

Marine being reported on has had an opportunity to make either a<br />
