TRICHOLEUKEMIA<br />

LABORATORIAL TESTS<br />

- Full hemogram with reticulocytes and hematoscopy of peripheral blood<br />

- Full biochemistry with hepatogram<br />

- Immunophenotype of peripheral blood<br />

- Cytochemistry of SP (acid phosphates resistant to tartrate +)<br />

- Bone marrow aspirated -> usually dry<br />

- Bone marrow biopsy with immunohistochemistry<br />

- Positive markers that differentiate from other B-lymphoproliferative diseases: CD11c, CD105, CD25<br />

TREATMENT<br />

- 1 st CHOICE: Cladribine 0.1mg/kg/day/7 days continuous infusion. Repeat after 6 months in case of<br />

absence of remission with the 1 st cycle<br />

- The subjects must receive prophylaxis to PCP with Bactrim up to 6 months after the conclusion of the<br />

treatment. Discontinue Bactrim during the days of cladribine infusion.<br />

RELAPSE / OPTIONS<br />

- Retreatment with the initial scheme<br />

- Usage of other purine analogous (e.g.: pentostatin)<br />

- INF<br />

- Rituximab<br />

- Consider splenectomy only in cases of symptomatic bulky splenomegaly, with no response to the drug<br />

treatment<br />
