SPECIAL PROCEDURES<br />

1 – INDICATIONS OF WASHED HEMOCOMPONENTS<br />

Washed or deplasmatized hemocomponents – they are erythrocitary or platelet components, from which<br />

most part of plasma was removed. They are indicated for:<br />

Subjects with a proved absence of immunoglobulin IgA<br />

Subjects with severe allergic reactions.<br />

2 – INDICATIONS OF IRRADIATED HEMOCOMPONENTS<br />

In order to reduce the risk of Host-versus-Graft-Disease (DECH), the hemocomponents must be irradiated.<br />

The irradiation dosage is 25 grays and must be performed in own cells irradiators for this purpose.<br />

Irradiated components are indicated:<br />

- At the intra-uterine transfusion<br />

- Subjects undergone an autologous or allogenic Bone marrow Transplantation, for the period of six (06)<br />

months to 1 year, respectively, from the day of the conclusion of pre-transplant conditioning regime.<br />

- During the bone marrow or stem cells collection of the peripheral blood in subjects who will be submitted<br />

to autologous TMO, from the moment in which the mobilization is started until the conclusion of the last<br />

collection.<br />

- Transfusion of blood or components donated by 1 st -degree relatives of the receptor.<br />

- Transfusion of blood or components collected from donors who present a HLA-compatibility to the<br />

receptor.<br />

- Transfusion in premature with a weight below 1,200 g<br />

- The heating of blood bags must be performed only at heaters suitable for this purpose.<br />

3 - INDICATIONS FOR THE HEAT BLOOD TRANSFUSION<br />

- Transfusion in premature new born<br />

- Fast transfusion (Flow above 50 ml/min)<br />

- Transfusion in poly-traumatized subjects<br />

- Transfusion at Cold Self-Immune Hemolytic Anemia<br />

In such cases, you must heat the subject, covering him with a blanket, and putting thick gloves and socks.<br />

Transfuse if there are any signs of fast hemolysis in the course of the transfusion, the subsequent<br />

transfusions must be done using the blood heater. Whenever there is an indication to use the blood or<br />

heat components, execute this procedure in suitable heaters for this purpose.<br />

4 - INDICATIONS FOR THE USAGE OF CMV-NEGATIVE BLOOD<br />

It is indicated in the following subjects’ categories (regardless their serologic status):<br />

- Undergone a Bone marrow Transplantation<br />

- Premature below 1,200 g<br />
