Conventional dental hand-pieces (high speed drills) run at 350,000 revolutions per minute creating a vacuum that pulls the odontoblasts out of the dentinal tubules (picture 3 and 4 above). Consider that dentinal tubules extend from under the enamel into the soft pulp chamber inside the tooth (picture 2). This is a serious problem because, without odontoblasts the tooth is unable to repair or create dentinal matter. In a healthy, undamaged tooth, a tubule carries nutrients from the pulp chamber in the center of the tooth, into the dentine of the tooth. In a damaged dentinal tubule, bacteria can invade the tooth, moving in the “wrong” direction, passing through the dentine and into the pulp chamber. Bacteria can then pass through these tubules directly into the nerve chamber of the tooth causing an abscess (infection) which will likely kill the tooth. Picture 5, a diagrammatic drawing of a vacuum system used at Seamen‟s Lab in Germany, demonstrates how use of the correct drill speed on the tooth reduces sensitivity and helps save teeth. HIGH SPEED DRILLING was my answer to my patient‟s question for the loss of her tooth. I learned this information too late to prevent high speed drilling from killing ten of my teeth. In 1987, I removed my high- speed drilling equipment from my office forever! 20,000 revolutions per minute has been my top speed for the past thirty years, making patients more comfortable and saving teeth that could have been lost due to high speed drilling. And, yes it does take longer to prepare a tooth for a crown, inlay, or composite restoration; however, I have reduced the number of teeth lost by my patients. It is a fact that every dentist can save more teeth by using low speed drilling. I would rather take more time with low-speed drilling and save teeth, than lose one tooth to save time with high-speed drilling. Patients with dying teeth from high-speed drilling are sent to endodontists and general dentists to perform unnecessary root canals.
