If a component was provided within 90 days be<strong>for</strong>e the Child Health Check-Up and the results were documented, the provider does not<br />

need <strong>to</strong> repeat the component. An exception should be made if the recipient’s environment or medical condition indicates that the<br />

component should be repeated. Or if, based on medical discretion, a component has not been per<strong>for</strong>med within approximately 90 days,<br />

but within reasonable time based on the recipient’s age and with documentation of medically appropriate rationale, i.e., 2-year old<br />
