This addendum is issued to modify the previously issued REQ documents and/or given for informational purposes, and is<br />
hereby made a part of the REQ documents. Please attach this addendum to the documents in your possession and<br />
acknowledge receipt of this addendum by signing and including this form in the response package.<br />
RFI’S:<br />
1: Please kindly clarify what information is sought in the field “Warranties” of Section 6.1 “Acknowledgment Form”<br />
Al: If your firm offers warranties of your services, please provide it.<br />
.2: Form 6.4 “RFQ Signature Page for Corporation” (Pg 41) requests we provide “full names and residences of<br />
stockbrokers, persons, or firms interested in the foregoing Bid, as principals...” Can you clarify which persons or irm<br />
should be provided in response to this request?<br />
A.2: The principals of the corporation, firm or organization submitting qualifications.<br />
Q.3: Page 34, Section 3.2 Scope of Services, paragraph 1 states: “The City may select multiple qualified persons or<br />
Firms for each category of specialized services as described in Form 330.” Will the City please clarify if this<br />
corresponds to the “List of Disciplines (Function Codes)” found on pg. 6 of the attached SF330? If not, will the City<br />
please advise where a comprehensive list of categories can be found?<br />

Is the SF330 Part I, Section F, also considered to be a reference since it requires a point of contact and Telephone<br />
number. It is clear that respondents cannot use NMB projects to satisfy the five (5) required in Section 6.3, however<br />
can respondents include previous experience with the City of North Miami Beach as one of the 10 projects included<br />
within Section F of the SF330 Part I?<br />
Yes, they can.<br />
It is understood that firms are limited to submitting a total of 10 projects (within Section F) per the SF330 part I. It is<br />
also understood that each team subconsultant is required to include an SF330 as well as references for the discipline<br />
of service they are anticipated to provide. As a result, a prime firm teamed with multiple subs will have the opportunity<br />
to include more than 10 projects overall. This appears to create a disadvantage to a single firm wishing to submit for<br />
multiple disciplines. Will the City consider allowing a single firm, submitting on multiple disciplines, to submit 10<br />
projects per discipline so that it would have ample opportunity to demonstrate relevant experience as would a firm<br />
responding to one discipline?<br />
Firms are allowed to submit 10 projects per discipline.<br />
Addendum 1; Q3 — Are sub-consultants required to submit City’s Reference Questionnaire form 6.3 as well as the<br />
Prime Consultant?<br />
