Addendum 3; Q7 — Form 330 is created on a government forms software with same information as noted on the RFQ
but slightly different look. Confirm that the government form software is also acceptable.
The City will accept this version as long as the same information required is submitted and if the differences are
cosmetic.
Is it admissible to use the SF 330 form issued by the GSA or is the only form permitted the one include in the REQ
package?
The form included in the RFQ package is issued by GSA. If you are refer~ng to the pdf version also issued by GSA,
then yes, the City will accept it.
We have reviewed item 1.36 Indemnification and Hold Harmless Agreement for REQ No. 2012-10 Continuing
Contract for Professional Services for North Miami Beach and find that it does not comply with Chapter 725 of the
Florida Statutes. Will the City modify its indemnification language to comply with Chapter 725.08 of the Florida
Statutes?
This question will be answered in a subsequent addendum.
Do we need to have a vendor number before submitting our qualification package?
No, your vendor registration form may be submitted along with your qualification package.

references directed to third parties could be used to satisfy this request.

In regards to the five references requested on form 6.3 Reference Questionnaire, should these five references
include one for our subcontractor or should all five references be for the prime?
Include five references questionnaires for the prime and five for your subcontractor.
Q.13: Are sub-section tabs allowed to be used in a firm’s submittal, beyond the listed sections on page 36.
