24. No clinic patient shall have a prescripti<strong>on</strong> re-dispensed at the next visit<br />

unless all <strong>drugs</strong> are re-prescribed item by item.<br />

25. Before re-issuing any <strong>drugs</strong>, it should be ensured that the patient has in<br />

fact taken regularly the <strong>drugs</strong> prescribed earlier.<br />

26. Drugs should not be issued to patients for a period <strong>of</strong> over four weeks.<br />

27. For patients discharged from indoor, <strong>drugs</strong> should be prescribed <strong>on</strong>ly for<br />

three days or to last till the next clinic date. In any event the issue should not<br />

be in excess <strong>of</strong> two weeks.<br />

Other Stock C<strong>on</strong>trol Procedures (applicable to both indoor and out door<br />

dispensaries)<br />

28. All OPD and Clinic prescripti<strong>on</strong>s should carry the name, age and the full<br />

postal address <strong>of</strong> the patient. The address should be with sufficient detail so<br />

that the patient could be located in the community.<br />

29. The dispensing <strong>of</strong>ficers should indicate the number <strong>of</strong> units <strong>of</strong> each drug<br />

issued <strong>on</strong> the prescripti<strong>on</strong>. If any drug is out <strong>of</strong> stock, this should be indicated<br />

in the prescripti<strong>on</strong> and signed by the <strong>of</strong>ficer dispensing the drug.<br />

30. The dispensing <strong>of</strong>ficer should give clear instructi<strong>on</strong>s (both written and<br />

verbal) to the patients with regard to the use <strong>of</strong> <strong>drugs</strong> dispensed by them. In<br />

additi<strong>on</strong>, name <strong>of</strong> the drug should be written <strong>on</strong> the label <strong>of</strong> each drug<br />

dispensed.<br />

31.List <strong>of</strong> n<strong>on</strong> available and low stock <strong>drugs</strong> as well as the <strong>drugs</strong> that are<br />

found to be in excess/slow moving at the instituti<strong>on</strong> or at RMSD/MSD (if<br />

such informati<strong>on</strong> is available), should be brought to the notice <strong>of</strong> all<br />

prescribing <strong>of</strong>ficers at weekly intervals. Such informati<strong>on</strong> is to be displayed<br />

at appropriate places for easy attenti<strong>on</strong> <strong>of</strong> all prescribing <strong>of</strong>ficers. With regard<br />

to n<strong>on</strong>-available <strong>drugs</strong> and those in low stock, suitable alternatives/ substitutes<br />

available should also be brought to the notice <strong>of</strong> prescribing <strong>of</strong>ficers so that<br />

suitable substitutes can be prescribed. The resp<strong>on</strong>sibility to see that these are<br />

d<strong>on</strong>e lies with the Chief Pharmacist.<br />

1. The following formulati<strong>on</strong>s should be accounted for in detail, daily at<br />

points <strong>of</strong> use.<br />

i. Tablets and capsules costing Rs. 5.00 or more per unit.<br />
