7.0 Anti rabies pre-exposure therapy<br />
This form <strong>of</strong> therapy is indicated for pers<strong>on</strong>s who are at higher risk <strong>of</strong><br />
exposure to rabies virus ie laboratory staff handling live rabies virus,<br />
veterinarians, rabies c<strong>on</strong>trol staff( vaccinators) wild life <strong>of</strong>ficers, employees in<br />
animal quarantine premises and zoological establishments.<br />
The recommended schedule is IM-ARV- 1dose each <strong>on</strong> D0,D7& D28 A<br />
booster dose is given 1 year after the 1 st dose. Additi<strong>on</strong>al booster doses are<br />
given <strong>on</strong>ce every 5 years.<br />
Administrati<strong>on</strong> <strong>of</strong> RIG is c<strong>on</strong>traindicated in pers<strong>on</strong>s <strong>on</strong> pre-exposure therapy<br />
They should be given additi<strong>on</strong>al doses <strong>of</strong> IM-ARV 1 dose each <strong>on</strong> D0& D3 as<br />
boosters even in a case <strong>of</strong> a major exposure.<br />
8.0 Important points to be noted<br />
Suturing is best avoided. If necessary, should be d<strong>on</strong>e after infiltrati<strong>on</strong><br />
with RIG<br />
RIG should be administered before starting <strong>on</strong> AVR<br />
Avoid initiati<strong>on</strong> 2-1-1 schedule <strong>of</strong> IM-ARV or 8 site ID-ARV following<br />
RIG<br />
Administrati<strong>on</strong> <strong>of</strong> RIG or ARV <strong>on</strong> the buttocks is not recommended as<br />
absorpti<strong>on</strong> is poor<br />
In small children with multiple bites , if the volume is insufficient for<br />
infiltrati<strong>on</strong> in and around the wounds, dilute the RIG with sterile N. Saline<br />
up to double or 3 times.<br />
Human to human transmissi<strong>on</strong> <strong>of</strong> rabies has not been reported( except<br />
through corneal or organ graft)<br />
For any pers<strong>on</strong> who has had direct or indirect c<strong>on</strong>tact with a rabies patent<br />
PET is not recommended except in special situati<strong>on</strong>s( Gen Circular No; 01-<br />
22/2004 <strong>on</strong> “ Guidelines for procedure to followed in a case <strong>of</strong> death due to<br />
human rabies and disposal <strong>of</strong> the body)<br />
Laboratory c<strong>on</strong>firmati<strong>on</strong> <strong>of</strong> rabies should always be encouraged.<br />
In instituti<strong>on</strong>s where the animal is not vaccinated, encouraged the owner<br />
to vaccinate the animal c<strong>on</strong>cerned after the observati<strong>on</strong> period<br />
Pregnancy is not a c<strong>on</strong>tra indicati<strong>on</strong> for RIG and ARV therapy when<br />
indicated.<br />
