INSTITUTIONAL DEVELOPMENT
Introduction
The work of the Institutional Development Working Group is guided by the institutional
challenges identified in the Health Sector Strategic Plan 2003-2007. This section of the report
identifies the current situation and achievements, constraints and priorities for each of the four
strategies identified in the plan.
Current Situation and Achievements
Strategy 17 - Organization and management reform of structure, systems and
procedures of the Ministry of Health (MoH) to respond effectively to change.
There has been considerable progress made in this area during 2006. Based on the functional
analysis report and the approval by High Level Group, and with the technical support from
OPM, the incentive reform process began with the design of the Merit- Based Pay Initiative
(MBPI) scheme .The design was discussed in several meetings of the High Level Working
Group attended by representatives from CAR, MEF and Health Partners and finally was
agreed on 14 November, 2006. Based on the agreement, a draft operational manual for the
implementation was developed and it is in the final stage of review by Health Partners. The
funding mechanism for the MBPI Scheme was finalized and agreed in a High Level Working
Group and Health Partners meeting on 28, November, 2006 .The scheme will be funded
jointly by Government and Health Partners following the Sub Decree 98 .Based on the
agreement, a draft Memorandum of Understanding (MoU) was prepared and is in the final
stage of review by Health Partners. The scheme will be implemented to cover 160 positions at
the central MoH during 2007 and based on the lessons learnt and experience gained; the
scheme will be extended to cover 2 PHDs and 2 National Programmes
Under Institutional Development Programme, the sub sector (Primary Health Care; Hospital
Services; National Programmes and Central MoH) assessments were completed with the
technical support from OPM .The findings and recommendations were presented in national
level workshop during 19-20 October, 2006. Based on the feedback, IDP Synthesis report was
prepared incorporating all the essential elements of the sub sector reports and was presented
to IDWG in January, 2007.The IDP synthesis report is of the key documents for Mid Term
Review.
Standard staffing levels for referral hospitals and health centres have been developed and
these standards have been approved and disseminated and incorporated into the HR database
so that variances can be reported.
To address the mal-distribution of staff and attempt to attract staff to remote geographical
areas, the MoH follows strictly the Royal Government Circular on the implementation of
annual cadre plan and also identified the different difficult regions and made the interministerial
Prakas between MoH and MEF on the use of equity fund.
A Priority Mission Group in Kampong Trach District has been continued its implementation.
Further proposals for additional PMGs in Takeo province and in the North West region of
Cambodia have been prepared and submitted to CAR for consideration.
